Аннотация.
В статье представлены исследования с использованием нейропсихологического
подхода к оценке нарушений когнитивной сферы при нормальном и
патологическом старении, выполненные в ФГБНУ НЦПЗ. Описываются
методологические принципы и особенности нейропсихологических
исследований в школе А.Р. Лурии и использование их в
нейрогеронтопсихологии. Представлены результаты нейропсихологического
подхода к исследованию нормального старения, в том числе варианты
старения. Показана специфика нейропсихологических синдромов при раннем
и позднем вариантах болезни Альцгеймера с использованием качественного
и количественного нейропсихологического анализа нарушений психической
деятельности. Также выявлено сходство и различие в структуре
нейропсихологических синдромов у больных со смешанной
альцгеймеровско-сосудистой деменцией в сравнении с болезнью Альцгеймера
и сосудистой деменцией. В статье рассмотрены результаты катамнестического
клинико-нейропсихологического исследования больных с синдромом мягкого
когнитивного снижения (MCI), показаны прогностически благоприятные и
неблагоприятные варианты изменений когнитивной сферы у пациентов с
мнестико-интеллектуальным снижением в рамках указанного синдрома.

Введение
С начала 80-х годов ХХ века в медицине, биологии и психологии
существенно возрос интерес к проблемам старения. Это во многом
обусловлено процессом «постарения» населения во всех
странах. Современная демографическая ситуация во всем мире
характеризовалась и характеризуется опережающим ростом численности
людей пожилого и старческого возраста. Возрастание числа лиц позднего
возраста ставит перед обществом и наукой множество вопросов, связанных
с психологическими особенностями людей на позднем этапе жизни. Особенно
интенсивно развиваются исследования в контексте нейронаук, так как
особенности психики в позднем возрасте при нормальном физиологическом
и патологическом старении обусловлены, прежде всего, структурными
изменениями и перестройками в церебральном функционировании.
В это же время отечественными геронтопсихиатрами перед
нейропсихологами школы А.Р. Лурии была поставлена задача изучения
мнестико-интеллектуальной сферы при различных деменциях позднего
возраста. Эта задача начала решаться в исследованиях под руководством
к.п.н., доцента Н.К. Корсаковой — ученицы А.Р. Лурии,
а также ведущего отечественного геронтопсихиатра, д.м.н., профессора
С.И. Гавриловой. Клинической базой этих работ были гериатрические
отделения ФГБНУ НЦПЗ (директор — академик РАН, профессор
А.С. Тиганов). При этом дифференциальная диагностика и описание
синдромов когнитивного снижения при нейродегенеративных заболеваниях
проводилось в сопоставлении с нормальным физиологическим старением
[1; 5; 12; 15].
Нейропсихологический подход к исследованию нарушений психических
функций в школе А.Р. Лурии
Нейропсихологический подход ориентирован на рассмотрение когнитивной
сферы в целом в синдромальной совокупности всех высших психических
функций (ВПФ) (гнозис, праксис, оптико-пространственная деятельность,
речь, память, мышление, внимание) и их регуляторного обеспечения
(произвольная регуляция и нейродинамические параметры психической
деятельности). Синдромный анализ результатов психической деятельности
предполагает наличие существующего в норме общего звена в структуре
психической деятельности (нейропсихологический фактор по А.Р. Лурия),
нарушение которого приводит к закономерному сочетанию расстройств в
когнитивной сфере и позволяет описать иерархию расстройств в
синдромологическом оформлении. Каждый из нейропсихологических факторов
связан с работой определенной зоны мозга [10].
Многочисленные исследования показали, что реализация
нейропсихологического подхода к анализу и описанию когнитивных
особенностей при нормальном и патологическом старении наиболее
эффективно обеспечивается применением концепции А.Р. Лурии
о трех функциональных блоках мозга (ФБМ). Эта модель позволяет
рассматривать возрастные изменения в рамках единой методологии
и связанным с ней клинико-экспериментальным комплексом оценки
состояния высших психических функций [2; 3; 4; 11; 14].
Нейропсихологическое исследование нормального старения
К числу наиболее часто встречающихся и отмеченных практически
всеми исследователями возрастных симптомов относится прежде всего
замедление темпа деятельности (латентность), особенно на этапе ее
инициации (наиболее отчетливо это проявляется при извлечении
информации из систем хранения в памяти). Несколько позднее наступает
сужение объема психической активности в виде ограничения присущих
молодому возрасту возможностей одновременного многоканального
параллельного выполнения различных действий (трудности переключения
и распределения внимания). Эти симптомы могут сопровождаться замедлением
скорости переработки информации, в частности, при восприятии речи,
произносимой в быстром темпе.
Одним из ведущих возрастных симптомов являются ограничения в текущем
запоминании, обусловленные повышенной тормозимостью следов памяти под
влиянием вновь поступающей информации, снижение помехоустойчивости
текущего запоминания к отвлекающим факторам.
Все перечисленные когнитивные ограничения в соответствии с
нейропсихологическими представлениями связаны с функциональной
дефицитарностью первого блока мозга. Снижается уровень энергетического
обеспечения психической деятельности, преобладают тормозные процессы,
то есть изменяются общие фоновые составляющие активности человека.
Вторая составляющая нормального старения связана с ограничениями в
переработке пространственных характеристик информации. Достоверно
верифицированы затруднения стареющих людей в актуализации
зрительно-пространственных представлений при выполнении простого
рисунка (дом, стол, куб), в расстановке стрелок на часах без циферблата,
в зрительно-пространственной памяти при запоминании последовательности
контурных фигур и др. Многочисленные данные об ограничениях в
зрительно-пространственной сфере свидетельствуют о снижении степени
участия правого полушария в когнитивных процессах. При этом наиболее
отчетливо представлена дисфункция височно-теменно-затылочной
ассоциативной области.
Одновременно возникают проблемы и с параллельной многоканальной
переработкой информации, поскольку интегрированное во времени выполнение
различных задач не только требует распределения и переключения внимания
(первый ФБМ), но и опирается на присущий правому полушарию принцип
симультанности. Таким образом, одной из составляющих когнитивного
старения является изменение вектора активности полушарий мозга в сторону
доминирования левого полушария [6; 7]. Функциональная сохранность
левого полушария (его компенсаторные возможности) может рассматриваться
как фактор-протектор от возможного развития ослабоумливающих процессов [7].
Н.К. Корсакова выделяет варианты нормального старения. Эта
типология основана на выявлении наиболее слабого и сензитивного к
нагрузке в клинико-психологическом эксперименте нейропсихологического
фактора, связанного с функциями первого, второго или третьего ФБМ [4].
В основе первого (наиболее часто встречающегося) варианта старения
лежит снижение уровня энергетического обеспечения активности. Характерным
является достаточно длительная латентность при включении в деятельность,
относительно короткий период ее эффективного выполнения, за которым
следует истощаемость и, как следствие, незавершенность действий. Кроме
того, имеют место повышенная тормозимость следов памяти, сужение объема
психической деятельности (одноканальность переработки информации).
Декомпенсация может наступить вследствие длительного дистресса,
чрезмерных физических и эмоциональных нагрузок, невозможности
регламентировать активность. При этом сохранность критичности,
регулирующей роли речи, наличие стратегий, обеспечивающих
распределенность активности во времени и внутреннем пространстве,
является хорошей предпосылкой для сохранного психического
функционирования даже при очевидном снижении энергетического ресурса.
Второй вариант старения связан с дефицитом симультанности в
переработке информации, что в первую очередь проявляется в
зрительно-пространственной сфере. Этот вариант старения может
рассматриваться как наиболее благополучный, особенно при
целенаправленном использовании стратегии «дробления»
процедур решения разных задач, перевода действий на уровень
развернутого, поэтапного, контролируемого, последовательного
выполнения. Декомпенсация при этом варианте старения может наступить
при резком и значительном изменении пространства и
«квазипространства» жизни, например, при смене места
жительства и среды обитания, лингвистической среды.
Третий вариант старения характеризуется снижением произвольной
регуляции деятельности в таких ее звеньях, как прогнозирование
результата, построение программ, адекватных задаче и своим
возможностям. Могут наблюдаться импульсивное принятие решений,
несоразмерность планов и неадекватная оценка доступности их
реализации по отношению к собственным возможностям, в том числе и
по времени достижения целей. Это при сохранности когнитивной
критичности может приводить к переживанию неуспеха и к
декомпенсации.
Неблагоприятен в плане прогноза декомпенсации четвертый (сочетанный)
вариант старения, где наиболее уязвимыми являются два фактора:
энергетического обеспечения активности и ее произвольной регуляции.
Модус функционирования таких людей отличается недостаточной спонтанностью,
сужением сферы интересов, эмоциональной монотонностью в сочетании с
повышенной забывчивостью. Декомпенсация может наступить при любых
изменениях среды, исключающих использование сложившихся, ранее привычных
форм активности. Этот вариант старения усугубляется и трансформируется
в деменцию в условиях соматического заболевания и депривации общения.
Нейропсихологическое исследование болезни Альцгеймера
Нейропсихологическое исследование когорты больных с пресенильным
(78 больных) и сенильным (87 больных) вариантом болезни Альцгеймера
(БА) с применением шкалы количественной оценки результатов
нейропсихологического обследования (адаптированная методика
А.Р. Лурии) показало следующее [5; 8; 11; 15].
Нейропсихологический синдром при пресенильном типе БА складывается
из двух основных составляющих. Первая из них — это нарушение
фоновых компонентов психической активности (снижение уровня активности
глубинных структур мозга): замедленность, истощаемость, отчетливо
выраженное проактивное торможение, представленное феноменом
преимущественного припоминания первых элементов стимульной
последовательности, в сочетании с патологическим негативным влиянием
гетерогенной и гомогенной интерферирующей деятельности на последующее
воспроизведение при запоминании любых стимулов, расстройства памяти
носят модально-неспецифический характер и приближаются по своим
механизмам к амнестическому синдрому. Вторая составляющая синдрома
включает такие симптомы, как нарушение оптико-пространственных функций,
импрессивной и номинативной функции речи, кинетической, кинестетической
и пространственной организации движений, зрительного гнозиса
в задачах на выделение фигуры из фона. На основании оценки речи и
связанных с ней процессов письма, чтения и счета можно говорить о
вовлеченности в патологический процесс широкой зоны левого полушария
мозга в той части его структурно-функциональных составляющих, которые
входят во второй блок мозга (теменные, теменно-затылочные, премоторные
отделы, а также внеядерные отделы левой височной доли мозга). Третий
блок мозга (лобные префронтальные отделы) на начальном этапе
пресенильного типа БА остается более функционально сохранным, чем
первый и особенно второй. Такая гетерохронность приводит к достаточно
длительному сохранению «фасада личности», некоторых
регуляторных функций, эмоциональной реакции на заболевание и
доступность ряда хорошо автоматизированных, закрепленных в прошлом
опыте навыков [1; 6; 8; 15].
Иную структуру имеет нейропсихологический синдром при сенильном
типе БА. Первая составляющая синдрома свидетельствует о дисфункции
первого энергетического блока мозга (сужение объема психической
деятельности или ограничения в ее многоканальности, нарушения памяти
в связи с патологической тормозимостью следов интерферирующими
воздействиями, нарушение нейродинамических составляющих в моторной
сфере).
Вторая составляющая синдрома представлена комплексом симптомов,
свидетельствующих о снижении функции произвольного контроля в
различных высших психических функциях (ВПФ): эхопраксии, сокращение
или потеря программы начатого действия, наличие контаминаций,
парафазий и конфабуляций в задачах на запоминание и воспроизведение,
замена целостного и обобщенного понимания сюжетной картинки
фрагментарным перечислением ее деталей, парагнозии, импульсивность
при выполнении проб «Реакция выбора», снижение
контролирующей и регулирующей роли речи в построении программ
движений и действий. Названные симптомы связаны с нарушениями в
работе префронтальных отделов (третий блок) мозга.
С описанными расстройствами сочетаются такие специфические симптомы,
как лицевая агнозия, негрубо выраженные акустико-мнестическая и
амнестическая афазии, трудности определения времени на «слепые»
часах, дефицитарность в оптико-пространственной деятельности в виде
плоскостного рисунка и неточного расположения деталей относительно
друг друга.
Перечисленные симптомы указывают на диффузную церебральную патологию
с преимущественным вовлечением третьего и первого блоков мозга при
относительной сохранности второго. Симптомы дисфункции последнего
имеют в основном правополушарную локализацию [1; 6; 8; 11; 15].
Сопоставление описанных синдромов позволяет выделить общее и
специфическое в нарушении психических функций и функционировании
мозговых структур при пресенильном и сенильном типах БА. К общим
расстройствам относятся дефекты фоновых компонентов психической
активности, обусловленные снижением уровня ее энергетического
обеспечения и изменением нейродинамических параметров работы мозга.
Это проявляется в сужении объема всех видов психической деятельности,
замедленности, застревании на отдельных фрагментах выполняемых заданий,
патологической тормозимости следов памяти в условиях интерференции,
в непродуктивности заучивания стимульного материала. Все эти симптомы
при сенильном типе БА выражены в меньшей степени, чем у больных с
пресенильным типом БА. Противоположная картина наблюдается в отношении
произвольной регуляции деятельности, которая нарушена у обеих групп
больных. Однако у больных с сенильным типом БА обнаруживается более
высокая степень ее дефицитарности и более широкий спектр соответствующих
симптомов. У больных с сенильным типом БА при репродукции следов
памяти наблюдается значительное число контаминаций и в ряде случаев
конфабуляций, что указывает на нарушение контроля за деятельностью.
Для этой группы больных характерно нарушение регулирующей роли речи,
снижение критичности, отсутствует переживание своей несостоятельности.
Все перечисленное означает, что симптомы, обусловленные нарушением
в работе третьего блока мозга, представлены при сенильном типе БА
в более развернутом виде.
Таким образом, в картине когнитивных расстройств при БА можно
выделить, с одной стороны, симптомы, характерные для нормального
старения (снижение функций первого ФБМ). Они представлены в наиболее
выраженной степени и претерпевают дальнейшее развитие в ходе болезни,
становясь все менее доступными компенсации. С другой стороны,
существенно отличающим БА от нормы является нарушение функций,
обеспечиваемых левой гемисферой мозга. Можно видеть, что при
нарастании клинической симптоматики сильно снижаются возможности
регуляции деятельности (3-й ФБМ), что приводит к полной
несостоятельности больных.
Нейропсихологическое исследование смешанной деменции
В группе деменций позднего возраста выделяется заболевание,
клинические признаки которого определяются сочетанием проявлений,
характерных для болезни Альцгеймера (БА) и сосудистой деменции
(СоД). Для его обозначения используются термины сочетанной,
смешанной или атипичной деменции. Смешанная деменция диагностируется
в тех случаях, когда при наличии клинических или анатомических
признаков БА и сосудистого ишемически-деструктивного мозгового
процесса невозможно объяснить клиническую картину или особенности
течения процесса, исходя из концепции одного заболевания
[9; 14; 16; 18]. Место прежних представлений о комбинации двух видов
деменции занимает современная концепция сочетания болезни Альцгеймера
с церебрально-сосудистым заболеванием [19; 21; 22]. В МКБ-10 диагноз
сочетанной альцгеймеровско-сосудистой деменции имеет отдельный код
— F00.2.
Проведено клинико-нейропсихологическое исследование с опорой на
описанный выше подход трех групп больных. Клинический материал
составили 283 больных (81 мужчина и 202 женщины) в возрасте 48—93
лет (средний возраст 73 года). В основную группу включены 94 пациента
(33%) с диагнозом деменции смешанного (альцгеймеровско-сосудистого)
генеза. В группы сравнения вошли 75 больных (27%) с диагнозом сосудистой
деменции и 114 больных (40%) с диагнозом деменции при болезни Альцгеймера
без клинических признаков церебрально-сосудистого заболевания.
Качественный анализ результатов нейропсихологического исследования
больных на этапе мягкой деменции при смешанной альцгеймеровско-сосудистой
деменции показал следующее. При исследовании зрительного предметного
гнозиса (узнавание перечеркнутых и наложенных изображений) наблюдались
трудности выделения фигуры из фона и латенция деятельности. Узнавание
букв и цифр в обычных и сенсибилизированных условиях (зашумленные
изображения) не вызывало трудностей. При исследовании неречевого слуха
в пробах на слухомоторные координации отмечавшиеся ошибки в целом были
обусловлены сужением объема деятельности и снижением контроля протекания
деятельности. Исследование соматогнозиса показало, что у большинства
больных наблюдались трудности выполнения заданий с закрытыми глазами.
При исследовании различных составляющих праксиса наблюдалась
относительная сохранность кинестетической организации праксиса
(достаточно сохранное выполнение проб на праксис позы пальцев рук
со зрительным контролем). При этом перенос поз с одной руки на другую
без зрительного контроля у всех пациентов вызывал трудности, что
указывает на нарушение фактора межполушарного взаимодействия. Также
у всех больных отмечалось нарушение кинетической организации
произвольных движений (выраженные трудности выполнения пробы
«кулак-ребро-ладонь»). При исследовании пространственной
организации праксиса (пробы Г. Хеда) наблюдался большой разброс
результатов, при этом у большинства больных наблюдались пространственные
и зеркальные ошибки. Произвольная регуляция праксиса («реакции
выбора») у всех пациентов оказалась нарушенной.
Исследование оптико-пространственной деятельности (самостоятельный
рисунок, определение времени на «слепых» часах и др.)
показало существенное снижение в этой сфере у всех пациентов при
большом разбросе результатов.
При выполнении проб на фонематический слух затруднений не
наблюдалось, также как и при исследовании кинестетической
составляющей речевой моторики. У большинства пациентов отмечались
трудности повторения сложных слов и скороговорок, что указывает на
снижение кинетической организации речи. У всех обследованных имело
место в разной степени выраженное снижение номинативной функции речи,
а также значительное нарушение семантической организации речи. У
большинства наблюдалось нарушение письма (чаще в связи со снижением
кинетической составляющей этой деятельности), при этом чтение
оставалось относительно сохранным. Результаты исследования счетных
операций показали относительную сохранность автоматизированного счета,
при выполнении произвольного счета ошибки были обусловлены трудностями
удержания программы и снижением контроля выполнения задания.
Исследование памяти показало, что у большинства больных нарушения
памяти носили модально неспецифический характер и были обусловлены
сужением объема запоминания, патологической тормозимостью следов при
воспроизведении, а также нарушением избирательности (побочные ассоциации).
У некоторых пациентов отмечались различия в результатах выполнения
заданий на слухоречевую и зрительную память (результаты по слухоречевой
памяти были хуже). Запоминание семантически организованного материала
(рассказ, предложение) у всех пациентов вызывало существенно меньшие
трудности, чем заучивание по смыслу не организованных стимулов (ряд
слов, ряд фигур). Узнавание во всех модальностях было относительно
сохранно.
При выполнении интеллектуальных проб («сравнение понятий»,
«4-й лишний», понимание пословиц, решение арифметических
задач и др.) было обнаружено, что многим пациентам доступно оперирование
обобщенными признаками, однако у всех отмечались ошибки, обусловленные
импульсивностью, снижением контроля, потерей программы деятельности.
Понимание сюжетных картинок (особенно последовательных картинок)
снижалось из-за фрагментарности восприятия, элементов «полевого
поведения».
Оценивая нейродинамические составляющие психической деятельности,
следует отметить, что у всех пациентов наблюдалась (в разной степени)
латенция включения в отдельные задания, колебания продуктивности,
общая замедленность и дезавтоматизация деятельности, трудности
переключения в отдельных заданиях.
Также у всех обследованных наблюдались отчетливые симптомы со
стороны передних лобных структур мозга, а именно потеря (сокращение)
программы в процессе ее выполнения, выполнение заданий при условии
речевой регуляции со стороны психолога, вторичная (с помощью исследователя)
коррекция допускаемых пациентом ошибок.
Таким образом, качественный синдромный анализ результатов
нейропсихологического исследования позволяет выделить основные
составляющие синдрома нарушения ВПФ при мягкой смешанной деменции.
Первая составляющая синдрома — нарушение нейродинамических
параметров деятельности (1-й блок мозга, глубинные структуры),
второй компонент синдрома — симптомы со стороны лобных структур
мозга (3-й блок мозга). На этом фоне — отчетливое снижение
функций со стороны задней ассоциативной зоны мозга, входящей во
2-й блок мозга.
Количественная обработка результатов нейропсихологического
обследования предполагает суммирование оценок симптомов со стороны
каждого из трех блоков мозга. Общая оценка симптомов со стороны
1-го блока мозга состояла из оценки активационного обеспечения
деятельности, динамических параметров деятельности и прочности
запоминания. Суммарная оценка работы 2-го блока мозга включала
оценки зрительного и слухового гнозиса, кинестетического и
пространственного праксиса, оптико-пространственной деятельности,
номинативной функции речи, операциональной стороны мышления, письма,
объема запоминания 10 слов. Оценка симптомов со стороны 3-го блока
мозга складывалась из оценки контроля, программирования, произвольной
регуляции деятельности и кинетической организации праксиса и речи.
Сравнение результатов количественной оценки нейропсихологического
исследования (суммарных параметров со стороны трех блоков мозга) при
смешанной деменции в сравнении с болезнью Альцгеймера и сосудистой
деменцией показало наличие значимых различий между группами пациентов
на этапе мягкой деменции по всем трем показателям.
Наиболее выраженное нарушение нейродинамических параметров психической
деятельности наблюдалось у больных с сосудистой деменцией, наименьшее
снижение — у больных с деменцией альцгеймеровского типа, больные
с деменцией смешанного генеза по этому параметру занимали промежуточное
положение.
Оценка произвольной регуляции деятельности показала, что этот
параметр деятельности был более нарушен у больных с деменцией
альцгеймеровского типа и наиболее сохранен у больных с сосудистой
деменцией, больные со смешанной деменцией и по данному параметру
занимали промежуточное положение.
Оценка операциональных составляющих психической деятельности
показала большое разнообразие симптомов нарушения гнозиса, праксиса,
оптико-пространственной деятельности, речи, письма, счета, чтения,
интеллектуальных операций у больных всех трех групп, однако у больных
с сосудистой деменцией в целом имела место большая сохранность
операциональных компонентов психической активности.
Таким образом, по суммарным показателям симптомов со стороны всех
трех блоков мозга смешанная деменция занимала промежуточное положение
между болезнью Альцгеймера и сосудистой деменцией. Следует отметить,
что аналогичные результаты были получены при сопоставлении больных на
этапе умеренной деменции, однако эти различия не достигали уровня
статистической значимости. Можно говорить о том, что дифференциальная
нейропсихологическая диагностика наиболее эффективна на этапе мягкой
деменции.
Катамнестическое клинико-нейропсихологическое исследование пациентов
с синдромом мягкого когнитивного снижения
Современное состояние исследований и научно-практических разработок
в геронтологии и гериатрии характеризуется направленностью на поиск
фармакотерапии когнитивного снижения при старении. В этом контексте
задача идентификации проявлений прогрессирующего когнитивного дефицита
и отграничение этих состояний от когнитивного снижения непрогредиентного
характера приобретает особую актуальность и практическую значимость.
Создана исследовательская концепция «mild cognitive impairment»
(MCI — мягкое когнитивное снижение) для диагностического выделения
категории пациентов с таким ухудшением когнитивных функций, которое
представляет собой промежуточную стадию между возрастной нормой и
деменцией и имеет (или не имеет) риск перехода в деменцию в течение
ближайших лет. Речь идет о необходимости сделать объектом превентивной
терапии наиболее раннюю (доклиническую) стадию развивающейся деменции.
Особую важность в связи с тенденциями прогрессирующего роста числа
пациентов приобретает распознавание MCI предположительно альцгеймеровского
типа [13; 14; 17; 20]. Поскольку большинство исследователей сходятся
во мнении о том, что MCI является гетерогенным и по критериям оценки,
и по клинической картине, необходима систематизация этих расстройств
с учетом сложного многоуровнего строения выявляемых симптомов и их
мозговой отнесенности.
Имеются результаты применения структурно-функциональной модели
мозга А.Р. Лурии для диагностики когнитивного снижения у
пациентов с MCI и динамики нарастания симптомов мнестико-интеллектуальной
сферы в течение 2-х летнего катамнеза [13]. Было обследовано 40
пациентов (средний возраст — 69,3 ± 7,2).
Все пациенты, включенные в когорту наблюдения, были обследованы по
схеме оценки состояния ВПФ, выполнявшейся в традициях отечественной
нейропсихологии. Количественная обработка результатов
нейропсихологического исследования проводилась с применением шкалы
количественной оценки нарушений ВПФ у больных позднего возраста.
Качественный и количественный синдромный анализ результатов
нейропсихологического исследования позволил выделить основные
составляющие нейропсихологического синдрома снижения ВПФ и их
иерархию у пациентов с MCI. Исследование проводилось дважды с
интервалом в 2 года, в течение которого все пациенты получали
поддерживающую терапию. Сравнение структуры нарушений ВПФ при
первичном и катамнестическом обследовании позволяет сделать
выводы о нейропсихологических критериях прогноза МСI на основе
концепции ФБМ.
Через два года после первичного обследования когорта пациентов
распределилась на две подгруппы. В первую подгруппу вошли 23 человека
(57,5%), у которых не наблюдалось отрицательной динамики ВПФ. При
этом из них почти 60% при первом обследовании были отнесены к
«энергетическому» варианту (1 блок мозга) нормального
старения [4; 8] с незначительным усилением тормозимости следов
памяти и сужением объема психической деятельности. Вторая часть этой
стабильной подгруппы (40%) имела дефициты симультанности в переработке
информации (в большей степени зрительно-пространственной), т.е.
соответствовала варианту старения с ограничениями в уровне
функционирования 2 блока мозга.
Таким образом, возрастное когнитивное снижение, связанное с
дисфункцией 1 и 2 блоков мозга, не характеризуется прогредиентностью.
Констатация стабилизации состояния пациентов является важным основанием
для реализации программ психологической поддержки и сопровождения.
Этих пациентов достаточно информировать о «слабых»
звеньях ВПФ и предложить им стратегии ауторегуляции и смыслового
наполнения жизни на фоне поддерживающей фармакотерапии.
Иная картина имела место во второй подгруппе (42,5%), где уже
при первом нейропсихологическом обследовании выявлялись симптомы,
указывающие на импульсивность при решении арифметических задач и
в целом снижение контроля за деятельностью и ее произвольной
регуляцией. У 7 пациентов второй подгруппы наблюдалась отчетливая
дефицитарность со стороны лобных систем мозга. Вторая часть этой
подгруппы (10 пациентов) при первичном обследовании обнаруживала
в сочетании симптомы дисфункции 1 и 3 блоков мозга. В этой подгруппе
2-х летний период жизни пациентов, несмотря на поддерживающую терапию,
характеризовался нарастанием не только ранее выявленных симптомов, но
и общим когнитивным снижением и социальной дезадаптацией. Двум больным
на момент второго обследования был поставлен диагноз болезни Альцгеймера.
Приведенные данные нейропсихологического обследования дают основание
для организации когнитивных тренингов или специализированных коррекционных
программ с опорой на структуру слабых и сильных звеньев в психической
деятельности стареющего человека.

Abstract.
The article presents the studies based on neuropsychological approach
to the assessment of cognitive impairments in normal and pathological
aging carried out at MHRC. It describes the methodological principles
and specific features of neuropsychological researches in
A.R. Luria’s school and their use in neurogerontopsychology. It
represents the results of neuropsychological approach to the study
of normal aging including the variants of aging. The article shows
the specificity of neuropsychological syndromes in early and late
versions of the Alzheimer’s disease using qualitative and quantitative
analysis of neuropsychological disorders of mental activity. Besides,
it reveals similarities and differences in the structure of
neuropsychological syndromes in patients with mixed Alzheimer's-vascular
dementia, compared with Alzheimer's disease and vascular dementia.
The article describes the results of the katamnestic
clinical-neuropsychological study of patients with the syndrome of
mild cognitive impairment (MCI) and shows the prognostically favorable
and unfavorable variants of cognitive changes in patients with mnestic
and intellectual decline in the context of this syndrome.

Introduction
The interest to the problems of aging has significantly increased
in medicine, biology and psychology since the early 80s of the 20th
century. This is largely determined by the process of population aging
in all countries. Modern demographic situation in the world has been
characterized by outstripping growth in the number of elderly and
senile people. The increase in the number of people of late age puts
in front of science and society a lot of questions related to
psychological characteristics of people in later stage of life.
The studies in the context of neurosciences have been developing most
intensively, as the specific features of psyche in later age in cases
of normal physiological and pathological aging are determined first
of all by structural changes and rearrangements in cerebral functioning.
At that time, domestic neurogerontopsychiatrists gave
the neuropsychologists from A.R. Luria’s school a task to
study mnestic and intellectual sphere in cases of various dementias
of late age. This task began to be solved in the studies under
the guidance of Luria’s pupil, associate professor
N.K. Korsakova, PhD, and professor S.I. Gavrilova, MD,
the leading domestic gerontopsychologyst. The researches were
conducted at geriatric departments of MHRC (directed by RAS
academician, professor A.S.Tiganov). It should be noted that
differential diagnostics and description of cognitive decline syndroms
in cases of neurodegenerative diseases was carried out in comparison
with normal physiological aging [1; 5; 12; 15].
Neuropsychological approach to the study of the impairments of
mental functions in A.R. Luria’s school
Neuropsychological approach focuses on consideration of cognitive
sphere as a whole in the syndrome-based complex of all higher mental
functions (HMF) (gnosis, praxis, optical-spatial activity, speech,
memory, thinking, attention) and their regulatory provision (voluntary
regulation and neurodynamic parameters of mental activity). Syndromic
analysis of mental activity implies the presence of the existing in
norm common element in the structure of mental activity (neuropsychological
factor according to A.R. Luria), the violation of which leads to
a naturally determined combination of cognitive disorders and allows
to describe the hierarchy of disorders in the syndromological framework.
Each of neuropsychological factors is associated with the operation of
specific brain zone [10].
Numerous
studies
have shown that the usage of A.R.Luria’s concept of the three
functional blocks of brain (FBB) provides the most effective
realisation of neuropsychological approach to the analysis and
description of cognitive characteristics in cases of normal and
pathological aging. This model allows to consider age-related changes
within the framework of a unified methodology and related clinical and
experimental complex of assessment of higher mental functions
[2; 3; 4; 11; 14].
Neuropsychological study of normal aging
Slowing of the pace of activity (latency), especially at the stage
of its initiation (most clearly manifested when extracting the
information from storage systems in memory) is one of the most frequent
age-related symptoms that are noted by almost all the researchers.
Later stages are characterized by the narrowing scope of mental activity
in the form of limitation of typical for the young age possibility of
simultaneous multi-channel parallel execution of different actions
(difficulties in switching and distribution of attention). These
symptoms may be accompanied by slow rate of information processing,
in particular the perception of speech pronounced at a rapid pace.
Restrictions in current memorization determined by increased
inhibitability of memory traces under the influence of the newly
incoming information and decrease of current memorization's
interference immunity to distracting factors are one of the main
age-related symptoms.
According to neuropsychological concepts, all these cognitive
limitations are associated with the functional deficiency of the first
block of brain. The level of energy provision of mental activity goes
down, inhibitory processes prevail, i.e., general background components
of human activity change.
The second component of normal aging is associated with the
limitations in the processing of spatial characteristics of information.
It has been reliably verified that aging people have difficulties with
the actualization of the visual-spatial concepts while executing simple
drawings (house, table, cube), with placing the hands on the clock
without dial, with visual-spatial memory while memorizing the sequence
of contour shapes etc. Numerous data about the limitations in
visual-spatial field show that the degree of involvement of the right
hemisphere in cognitive processes decreases. At that, the dysfunction
of temporal-parietal-occipital association area is represented most
clearly.
The problems with multi-channel parallel processing of information
arise as well, since integrated in time performance of various tasks
not only requires the distribution and switching of attention (the first
FBB), but also bases itself upon the principle of simultaneity inherent
to the right hemisphere. Thus, the change of the activity vector of
cerebral hemispheres in the direction of dominance of the left hemisphere
is one of the components of cognitive aging [6; 7]. Functional
preservation of the left hemisphere (its compensatory possibilities)
can be considered as a protecting factor that prevents possible
development of processes leading to dementia [7].
N.K. Korsakova identifies
variants of normal aging. This typology is based on the detection of
neuropsychological factors associated with the functions of the first,
second or third FBB, which are the weakest and the most sensitive to
loadings in clinical psychological experiment [4].
The first (and the most frequent) variant of aging is based on
the reduced level of energy provision of activity. It is characterized
by rather a long latency when involved into activities, a relatively
short period of its effective implementation followed by exhaustion
and, as a consequence, incompleteness of actions. In addition, there
is an increased inhibitability of memory traces and narrowed scope of
mental activity (single channel of information processing). Decompensation
may occur due to prolonged distress, excessive physical and emotional
loads and impossibility to regulate activity. It should be mentioned
that preservation of criticality, the regulatory role of speech, the
presence of strategies that provide the distribution of activity in
time and inner space is a good prerequisite for intact mental
functioning, even at obvious decrease of energy resources.
The second variant of aging is associated with a deficit of
simultaneity in information processing, which is primarily manifested
in visual-spatial field. This variant of aging can be regarded as the
most favorable, especially if one purposefully uses the strategy of
procedures "fragmentation" for solving various tasks and switch-over
of actions to the level of full-scale, phased, controlled and sequential
execution. Decompensation in this variant of aging may occur in case of
a sudden and significant change of space and "quasispace" of life, for
example, when changing the place of residence and life environment,
linguistic environment.
The third variant of aging is characterized by a decrease in voluntary
regulation of activity in such of its elements as forecasting of the
result and building of programs that are adequate to the task and
one’s capabilities. An impulsive decision-making, disproportion
of plans and inadequate assessment of the possibility of their
implementation in relation to one’s own capabilities, including
the time required to reach the goals, can be observed. This can lead
to the experience of failure and decompensation in case the cognitive
criticality is preserved.
The fourth (combined) variant of aging is unfavorable in terms of
prognosis of decompensation. In this case two factors, namely energy
provision of activity and its voluntary regulation are the most
vulnerable ones. Modus of functioning of such people is characterized
by insufficient spontaneity, narrowed sphere of interest and emotional
monotony in combination with increased forgetfulness. Decompensation
may occur at any changes in the environment, which exclude the use of
existing, previously familiar forms of activity. This variant of aging
exacerbates and transforms into dementia under the conditions of physical
illness and deprivation of communication.
Neuropsychological study of Alzheimer's disease
Neuropsychological study of the cohort of patients with presenile
(78 patients) and senile (87 patients) variant of Alzheimer's disease
(AD) using a scale of quantitative assessing of results of
neuropsychological testing (A.R. Luria adapted technique) showed
the following [5; 8; 11; 15]. Neuropsychological syndrome at presenile
type AD is composed of two main components. The first of them is an
impairment of background components of mental activity (reduction of
activity of deep brain structures): slowness, exhaustibility, clearly
expressed proactive inhibition represented by the phenomenon of
predominant recollection of the first elements of the stimulus sequence
combined with the pathological negative impact of heterogeneous and
homogeneous interfering activities at subsequent reproduction when
memorizing any stimuli; memory disorders have modal nonspecific character,
their mechanisms are similar to the amnestic syndrome. The second
component of the syndrome includes such symptoms as the impairment of
the optical-spatial functions, impressive and nominative function of
speech, kinetic, kinesthetic and spatial organization of movements and
visual gnosis in the tasks on singling out figures from the background.
One can speak about the involvement of the general area of the left
hemisphere of the brain in the part of its structural and functional
components, which are included in the second block of the brain
(parietal, occipital-parietal, premotor areas and extranuclear areas of
the left temporal lobe of the brain) in the pathological process on the
basis of the assessment of speech and the associated processes of
writing, reading and counting. The third block of the brain (prefrontal
areas) remains more functionally intact at the initial stage of
presenile type AD than the first and especially the second one. This
heterochrony leads to a rather long-term preservation of the
"personality facade", some regulatory functions, emotional response to
the disease and the availability of a number of well-automated skills
that are enshrined in the past experience [1; 6; 8; 15].
The neuropsychological syndrome in the senile type AD has another
structure. The first component of the syndrome indicates dysfunction
of the first energy block of brain (narrowing scope of mental activity
or limitations in its multichannelness, memory impairment due to
pathological inhibitability of traces caused by interfering effects
and impairment of neurodynamic components in the motor sphere).
The second component is represented by a complex of symptoms showing
a decrease in the function of voluntary control in a variety of higher
mental functions (HMF): echopraxias, reduction or loss of program of
the initiated action, the presence of contamination, paraphasias and
confabulations in the tasks on memorizing and reproduction, replacement
of a holistic and generalized understanding of the story picture by
fragmentary enumeration of its parts, paragnosias, impulsivity in case
of performing "Choice Reaction" test, reduction of controlling
and regulatory role of speech in arranging programs of movements and
actions. The above mentioned symptoms are associated with a dysfunction
of the prefrontal areas (third block) of the brain.
Such specific symptoms as facial agnosia, mildly expressed
acoustic-mnestic and amnestic aphasia, difficulties in determining time
by "blind" clock, deficit in the optical-spatial activity in the form
of a planar drawing and inaccurate arrangement of details relative to
each other are combined with the described disorders.
These symptoms point to diffuse cerebral pathology with predominant
involvement of the third and the first blocks of the brain with relative
preservation of the second one. Symptoms of dysfunction of the latter
one are localized mostly in the right hemisphere [1; 6; 8; 11; 15].
A comparison of the described syndromes allows to single out the
general and the specific in the impairment of mental functions and
functioning of brain structures in cases of presenile and senile
types of AD. General disorders include defects of background components
of mental activity caused by decrease in the level of its energy
provision and changes in neurodynamic parameters of functioning of
the brain. This is manifested in the narrowing scope of all types of
mental activity, slowness, fixedness on the particular fragments of
the executed task, pathological inhibitability of memory traces in
conditions of interference and unproductiveness of memorization of
stimulus material. All of these symptoms are less expressed in case
of senile type than in patients with presenile type AD. The opposite
pattern is observed in respect of voluntary regulation of activity
that is impaired in both groups of patients. However, patients with
senile type AD demonstrate higher degree of its deficiency and a
broader range of relevant symptoms. Patients with senile type AD show
a significant number of contaminations and in some cases confabulations
during the reproduction of memory traces, which indicates an impaired
control over the activity. This group of patients is characterized by
impaired regulatory role of speech and reduced criticality; they do
not feel anxious because of their incompetence.The above mentioned
means that the symptoms caused by the impairment in the third block
of the brain are presented in senile type AD in a more expanded form.
Thus, on the one hand, the symptoms associated with normal aging
(decreased function of the first FBB) can be distinguished in the
cognitive impairments pattern in case of Alzheimer's disease. They are
presented in the most expressed degree and undergo further development
in the course of the disease, becoming increasingly difficult to
compensate for. On the other hand, impairment of the functions provided
by the left hemisphere of the brain makes AD significantly different
from the norm. One can see that as clinical symptomatology increases,
the possibility to regulate activity (3rd FBB) greatly reduces, which
leads to a complete incompetence of patients.
Neuropsychological study of mixed dementia
The disease, clinical signs of which are determined by a combination
of manifestations typical for Alzheimer's disease (AD) and vascular
dementia (VD), is distinguished in the group of dementias of late age.
The terms of combined, mixed or atypical dementia are used as its
definition. Mixed dementia is diagnosed in the cases when in the
presence of clinical or anatomic signs of AD and vascular cerebral
ischemia-destructive process it is impossible to explain the clinical
pattern or peculiarities of the process basing on the concept of one
disease [6; 14; 16; 18]. A modern concept of a combination of
Alzheimer's disease and cerebrovascular disease takes the place of
previous ideas about the combination of two types of dementia [19; 21;
22]. The diagnosis of combined Alzheimer's-vascular dementia has a
separate code — F00.2 in ICD-10.
Clinical and neuropsychological study of the three groups of patients
has been carried out based on the above approach. Clinical material
comprised 283 patients (81 men and 202 women) aged from 48 to 93
(mean age is 73 years). The main group included 94 patients (33%)
diagnosed with dementia of mixed (Alzheimer's — vascular) genesis.
The comparison groups numbered 75 patients (27%) diagnosed with vascular
dementia, and 114 patients (40%) diagnosed with Alzheimer's dementia
without clinical signs of cerebro-vascular disease.
Qualitative analysis of the results of neuropsychological studies of
patients with mixed Alzheimer's and vascular dementia at the stage of
mild dementia showed the following. During the study of the visual
subject gnosis (recognition of crossed out and superimposed images),
difficulties in singling out the figure from the background and latency
of activity were observed. Recognition of letters and numbers in normal
and sensitized conditions (noisy picture) did not cause any difficulties.
During the study of nonverbal hearing in the tests on hearing-motoric
coordination, the registered errors were generally caused by the
narrowed scope of activity and the reduced control over the activity
course. The study of somatic gnosis showed that most patients had
difficulties in performing the tasks with their eyes closed.
During the study of various components of praxis, relative safety
of kinesthetic organization of praxis was observed (quite intact
carrying out of tests on praxis of postural fingers with visual control).
However, the transfer of poses from one hand to the other without visual
control caused difficulties in all patients, indicating the impairment
of interhemispheric interaction factor. Besides, impaired kinetic
organization of voluntary movements (expressed difficulties of performing
"fist-edge-palm" test) was registered in all patients. There was a
large scatter of results during the study of spatial organization of
praxis (G. Head’s test), and the majority of patients had
spatial and mirror errors. Voluntary regulation of praxis ("choice
reaction") turned out to be impaired in all patients.
Study of optical-spatial activity (independent drawing, determining
the time on the "blind" clock etc.) showed a significant decrease in
this field in all patients at large scatter of results.
When performing tests on phonemic hearing no difficulties were
observed, as well as during the study of kinesthetic component of
the speech motor skills. The majority of patients had difficulties in
repeating compound words and tongue twisters, which indicates a decrease
of kinetic organization of speech. All the examinees were characterized
by reduced nominative function of speech expressed in varying degrees,
as well as a significant impairment of semantic organization of speech.
The majority of patients showed impairment of writing (mostly due to
the decrease of the kinetic component of this activity), while reading
remained relatively intact. The results of the study of calculating
operations showed that automated counting was relatively intact, while
the errors during voluntary counting were caused by difficulties in
program retention and reduction of task execution control.
The study of memory showed that the impairments of memory in the
majority of patients had modal nonspecific character and were caused
by narrowed scope of memorization, pathological inhibitability of memory
traces during reproduction and impaired selectivity (incidental
associations). The differences in the results of tasks for the oral-aural
and visual memory (oral-aural memory results were worse) were registered
in some patients. Memorizing of semantically organized material (story,
sentence) caused significantly less difficulties in all patients than
overlearning of stimuli that were not arranged according to their meaning
(a train of words, a train of figures). Recognition was relatively
intact in all modalities.
Intellectual tests ("Comparison of Concepts", "4th Extra",
understanding proverbs, doing sums and others) showed that many
patients were able to operate with generalized signs, however all of
them demonstrated errors caused by impulsivity, reduced control and
loss of activity program. Understanding of the story pictures
(especially successive pictures) was reduced due to the fragmentation
of perception and elements of the "field behavior".
When assessing neurodynamic components of mental activity, it
should be noted that all patients (to varying degrees) had latency
of getting into certain tasks, productivity fluctuations, general
slowness and de-automatization of activity, difficulties of switching
in certain tasks.
Besides, all the examinees showed distinct symptoms from the part
of frontal lobes, namely the loss (reduction) of the program in the
course of its performing, performing of tasks on condition of speech
regulation by a psychologist, secondary (with the help of the researcher)
correction of errors made by a patient.
Thus, a qualitative syndromic analysis of the results of
neuropsychological study allows to identify the main components of
the syndrome of HMF impairments in soft mixed dementia. The first
component of the syndrome is the impairment of neurodynamic activity
parameters (1st block of the brain, deep structures), the second
component of the syndrome is represented by the symptoms from the part
of frontal lobes (the 3rd block of the brain). Against this background,
there is a clear reduction of functions from the rear association areas
of the brain included in the 2nd block of the brain.
Quantitative processing of the results of the neuropsychological
examination involves summation of symptom assessments from the part
of each of the three blocks of the brain. Overall assessment of
symptoms from the part of 1st block of the brain included assessing
of activation provision of activity, dynamic parameters of activity
and durability of memorization. Total assessment of the work of 2nd
block of the brain included the assessment of the visual and auditory
gnosis, kinesthetic and spatial praxis, optical-spatial activity,
nominative function of speech, the operational side of thinking,
writing and volume of 10 words memorizing. Assessment of symptoms of
the 3rd block of the brain consisted of assessing control, programming,
voluntary regulation of activity and kinetic organization of praxis
and speech.
Comparison of the results of quantitative assessment of
neuropsychological studies (summarized parameters from the three blocks
of the brain) in case of mixed dementia compared to Alzheimer's disease
and vascular dementia showed the presence of significant differences
between the groups of patients at the stage of mild dementia for all
three indicators.
The most expressed impairment of neurodynamic parameters of mental
activity was observed in patients with vascular dementia, the smallest
decline — in patients with dementia of the Alzheimer type;
patients with dementia of mixed genesis occupied an intermediate
position according to this parameter.
Assessment of voluntary regulation of activity showed that this
parameter of activity was more impaired in patients with dementia of
the Alzheimer type and best preserved in patients with vascular
dementia; mixed dementia patients occupied an intermediate position
according to that parameter as well.
Assessment of the operational components of mental activity showed
a wide variety of symptoms of gnosis, praxis, optical-spatial activities,
speech, writing, counting, reading and intellectual operations
impairments in patients of all three groups; however, operational
components of psychic activity in patients with vascular dementia
were, in general, better preserved.
Thus, mixed dementia occupies an intermediate position between
Alzheimer's disease and vascular dementia according to summarized
indicators of symptoms from all three blocks of the brain. It should
be noted that similar results were obtained during the comparison of
patients at the stage of moderate dementia; however those differences
did not reach statistical significance. We can say that the differential
neuropsychological diagnostics is most effective at the stage of mild
dementia.
Catamnestic clinical-neuropsychological study of the patients
with the syndrome of mild cognitive impairment
Modern studies and applied scientific developments in gerontology
and geriatrics focus on searching pharmacotherapy of cognitive decline
caused by aging. In this context, the task of identifying the
manifestations of progressive cognitive deficit and delimitation of
these states from cognitive decline of non-progredient nature gets a
special urgency and practical significance. The research concept of
"mild cognitive impairment" (MCI) has been established for diagnostic
singling out of the category of patients with such worsening of
cognitive function, which represents an intermediate stage between
the age norm and dementia and has (or does not have) the risk of
passing into dementia within the next few years. We are talking about
the necessity to make the earliest (preclinical) stage of developing
dementia an object of preventive therapy. Recognition of probable
Alzheimer's type MCI acquires particular importance as there are
tendencies of progressive increase of patients’ number [13;
14; 17; 20]. Since the majority of researchers agree that MCI is a
heterogeneous according to both assessment criteria and clinical
pattern, it is necessary to systematize these disorders taking into
account a complex multi-level structure of detectable symptoms and
their brain relatedness.
There are results of using of A.R. Luria’s
structural-functional model of the brain for the diagnosis of cognitive
decline in patients with MCI and the dynamics of growth of the symptoms
of mnestik and intellectual sphere during the 2-year catamnesis [13].
The study has involved 40 patients (mean age is 69.3 ± 7.2).
All the patients included in the observation cohort were examined
according to the scheme of assessment of the HMF state performed in
the best traditions of Russian neuropsychology. The cohort of patients
was divided into two subgroups in two years after the initial
examination. Qualitative and quantitative syndromic analysis of the
results of neuropsychological studies allowed to identify the main
components of neuropsychological syndrome of HMF decline and their
hierarchy in patients with MCI. The study was conducted twice with
2-year interval, during which all the patients received supportive
therapy. Comparison of the structure of HMF impairments during the
initial and catamnestic examination allows to draw conclusions about
the neuropsychological criteria for MCI prognosis based on the concept
of FBB.
The cohort of patients was divided into two subgroups in two years
after the initial examination. The first subgroup included 23 persons
(57.5%) without negative dynamics of HMF. It should be noted that
nearly 60% of them were referred to the "energy" variant (1st block of
the brain) of normal aging [4; 8] with a slight intensification of
inhibitability of memory traces and a narrowing scope of mental activity
at the first examination. The second part of this stable subgroup (40%)
had deficits in the simultaneity of information processing (mostly
visual-spatial), i.e. it corresponded to the variant of aging with
the limitations in the level of functioning of the 2nd block of
the brain.
Thus, the age-related cognitive decline associated with dysfunction
of the 1st and 2nd blocks of the brain is not characterized by
progredience. Ascertaining the stabilization of patients’ state
is an important basis for the implementation of programs of psychological
support and maintenance. It is sufficient to inform these patients of
the "weak" elements of HMF and offer them strategies of self regulation
and filling their life with meaning against the background of supporting
pharmacotherapy.
Another picture was observed in the second subgroup (42.5%), it is
at the first examination that neuropsychological symptoms indicating
impulsivity in solving arithmetic tasks and overall reduction of
control over the activity and its voluntary regulation were revealed.
7 patients of the second sub-group showed distinct deficiency from
the part of frontal brain systems. The second part of this sub-group
(10 patients) showed the symptoms of the dysfunction of both 1st and
3rd blocks of the brain during the initial examination. In this
subgroup, despite the supportive therapy, the 2-year period of
patients’ life was characterized not only by the increase of
previously identified symptoms, but also general cognitive decline and
social maladaptation. Two patients had been diagnosed with Alzheimer's
disease by the time of the second survey.
The above mentioned data of neuropsychological examination provide
the basis for arranging cognitive training or specialized correction
programs based on the structure of strong and weak elements of mental
activity of an aging person.
