Рефлексия как фактор защиты подростка в преодолении стрессовых воздействий дисфункциональной семейной системы
в условиях психосоматического расстройства

Аннотация

В статье представлено исследование системных дисфункций в семьях подростков в возрасте 11—14 лет. В исследовании приняло участие 77 подростков, среди которых 35 с психосоматическими расстройствами кишечно-желудочного тракта. Целью исследования явилось изучение специфики личностных особенностей, которые помогают подростку совладать с негативным стрессовым воздействием дисфункциональной семейной системы.

Дисфункции семейной системы и процессы сепарации подростка в семье изучались с помощью следующих методик: ЦТО (эмоциональный компонент идентификации), «исследование самооценки с помощью процедуры ранжирования», которая предоставляет информацию о когнитивной зависимости от родителей. «Анализ семейной тревоги» Э.Г. Эйдемиллера, В. Юстицкиса, а также опросник «Суверенность психологического пространства личности» С.К. Нартова-Бочавер, шкала семейного окружения (ШСО) — для оценки социального климата в семьях; шкала семейной адаптации и сплоченности (FACES-3). Теоретический анализ личностных характеристик подростка, снижающих риск соматизации позволил выделить рефликсию, уровень алекситимии и характер копинг-стратегий, которые изучались с помощью следующих методик: копинг-тест Р. Лазаруса и С. Фолкмана, опросник «Уровень рефлексивности» А.В. Карпова, Торонтская алекситимическая шкала, адаптированная в Институте им. В.М. Бехтерева.

Полученные результаты свидетельствуют о нарушении процессов сепарации подростка с психосоматическим расстройством в семье, повышенной эмоциональной зависимости от матери и депривации психологического пространства, высоком уровне семейной тревоги, несформированной иерархии правил, более низком уровне сплоченности и эмоциональных связей в семье. Установлена роль развивающейся рефлексии подростка как фактора защиты в преодолении стрессовых воздействий дисфункциональной семейной системы в условиях психосоматического расстройства.

Историческое развитие общества продолжает идти по пути расширения временных рамок подросткового возраста — периода между младшим школьным возрастом и окончательной психологической, социальной и материальной автономией человека. Одна из основных перемен, происходящих в это время, — смена положения подростка в семье, процессы сепарации, пересмотра и изменения семейных границ. Сложность вышеописанного процесса, кризис во многих сферах современного общества, ухудшение окружающей среды и другие причины, традиционно описываемые в данном случае, приводят к увеличению распространенности соматических заболеваний, в механизме возникновения которых большую роль играют психологические факторы. Распространенность психогенных соматических и соматогенных расстройств в детском и подростковом возрасте не только значительна, но и постоянно увеличивается. В настоящее время все большее число хронических заболеваний «молодеет».

В результате многолетних исследований подверженности детей и подростков заболеваниям, роли психологических и социальных вредностей в возникновении, провоцировании, формировании клинических проявлений, течения и исхода психосоматических расстройств, а также особенностей личности этих больных, их реакций на болезнь, жизненных ситуаций, возникающих в связи с заболеваниями, были сформулированы концепции механизмов развития психосоматических расстройств. К психосоматической медицине относятся работы Ф. Александера, В. Бройтигама, В.Д. Менделевича, Б.Д. Карвасарского, С.А. Кулакова, Д.Н. Исаева и многих др. Интегрируются существующие теории и рассматриваются психологические особенности, взаимосвязанные с возникновением того или иного заболевания в разные периоды жизни.

Отдельное место занимает блок теорий, утверждающих, что социальные факторы, имеющие начало в семейной среде, дают почву для хронического стресса и возникновения психосоматической патологии. К данным теориям относится и системное направление в семейной психологии, рассматривающее болезнь как дисфункциональный стабилизатор семейной системы. Концептуальное основание системного подхода к семье описано в работах представителей системной семейной психотерапии: М. Боуэна, В. Сатир, Г. Бейтсона, Э.Г. Эйдемиллера, В. Юстицкиса, А.В. Черникова, А.Я. Варги и др. Нормально функционирующая семья описывается авторами как семья, которая ответственно и дифференцированно выполняет свои функции, вследствие чего удовлетворяется потребность в росте и изменениях как семьи в целом, так и каждого его члена.

В современной психологии достаточно обширно представлены и теоретические обоснования, и экспериментальные исследования, подтверждающие влияние семьи на психическое и соматическое благополучие ребенка в ней. Однако нам представляется, что возможности личности ребенка, и особенно подростка (в силу развивающейся сепарации и автономии на этом возрастном этапе), в процессе совладания со стрессовым воздействием дисфункциональной семьи в условиях психосоматического расстройства изучены недостаточно.

Таким образом, нам необходимо исследовать специфические системные дисфункции, наблюдающиеся в семьях с подростком, страдающим психосоматическим расстройством, и лишь затем установить, существуют ли отличия в личностных особенностях здоровых и больных подростков, живущих в семьях с выявленными нами ранее дисфункциями.

Целью нашего исследования является изучение специфики личностных особенностей, которые помогают подростку совладать с негативным стрессовым воздействием дисфункциональной семейной системы.

Задачами нашего исследования являются:

1.   организация и проведение эмпирического исследования особенностей семейной системы у подростков, страдающих психосоматическими расстройствами (на примере заболеваний ЖКТ);

2.   на основе полученных результатов выделить критерии дисфункций семьи у подростков с заболеваниями желудочно-кишечного тракта;

3.   определить, какие личностные особенности способствуют преодолению негативного стрессового воздействия дисфункциональной семейной системы и будут взаимосвязаны с соматическим благополучием подростка.

Одними из основных задач, стоящих перед семейной системой в период взросления подростка, являются вопросы его отделения и развитие новых функциональных психологических границ в семье. Мы исходим из предположения, что нарушение процесса психологического отделения от родителей и, как следствие, отношения зависимости будут являться одними из главных компонентов нарушений семейной системы. Мюррей Боуэн писал, что при столкновении семьи и ребенка, с готовностью к сепарации последнего, семья «становится нестабильной и дезорганизованной, повышается внутрисемейное напряжение» [8]. С другой же стороны, заболевание будет оказывать воздействие на отношения и также может быть связано с нарушением этого процесса как реальное событие жизни. Так или иначе, согласно утверждениям некоторых авторов, проблемы сепарации и наличие психосоматического заболевания будут взаимосвязаны [1; 8; 9].

Таким образом, с нашей точки зрения, наиболее значимыми показателями функциональности семейной системы на рассматриваемом этапе развития семьи являются характеристики, описывающие процесс эмоциональной и когнитивной сепарации подростка от родителей, сохранности/депривированности психологических границ, «анализ семейной тревоги» как более общего показателя, свидетельствующего о неблагоприятном течении сепарации и процессах триангуляции. Под нарушением факторов семейной системы мы также, опираясь на теории Э.Г. Эйдемиллера, А.Я. Варги и др., понимаем нарушение семейной адаптации, семейной сплоченности и семейной коммуникации.

Для диагностики описываемых характеристик семейной системы мы использовали методику ЦТО (эмоциональный компонент идентификации); «Исследование самооценки с помощью процедуры ранжирования», которое предоставляет информацию о когнитивной зависимости от родителей; «Анализ семейной тревоги» Э.Г. Эйдемиллера, В. Юстицкиса, а также опросник «Суверенность психологического пространства личности» С.К. Нартовой-Бочавер; шкалу семейного окружения (ШСО) — для оценки социального климата в семьях; шкалу семейной адаптации и сплоченности (FACES-3).

Теоретический анализ личностных характеристик подростка, снижающих риск соматизации [2; 3; 5], позволил выделить следующие: развитие рефлексивности, уровень алекситимии и характер копинг-стратегий. Для решения данной задачи нашего исследования применялись следующие методики: копинг-тест Р. Лазаруса и С. Фолкмана, опросник «Уровень рефлексивности» А.В. Карпова, а также Торонтская алекситимическая шкала, адаптированная в Институте им. В.М. Бехтерева.

В исследовании приняли участие 77 подростков в возрасте 11—14 лет, 35 из которых имели диагнозы «Хронический гастродуоденит», «Неспецифический язвенный колит» (диагноз подростков подтвержден в отделении № 2 Курской областной детской больницы и ставился на основании данных анамнеза, жалоб, клинических показателей состояния ЖКТ, функциональных методов исследования), и 42 здоровых подростка со средней успеваемостью и адаптацией к школе. Исследование проводилось на базе областной детской клинической больницы и в школах г. Курска.

Для статистической обработки полученных результатов нами были использованы U-критерий Манна — Уитни, угловое преобразование Фишера φ* и коэффициент ранговой корреляции Спирмена rs.

Перейдем к обсуждению результатов исследования.

Решая первую из поставленных нами задач, мы сравнивали психологические характеристики семейных систем у больных и здоровых подростков. В результате данного сравнения были получены следующие результаты.

Обнаружены значимые различия между группами больных и здоровых подростков по всем показателям суверенности психологического пространства личности.

Суверенность физического тела (СФТ): p = 0,024.

Суверенность территории (СТ): p = 0,043.

Суверенность мира вещей (СВ): p = 0,003.

Суверенность привычек (СП): p = 0,049.

Суверенность социальных связей (СС): р = 0,0013.

Суверенность ценностей (СЦ): р = 0,012.

Интегральный показатель суверенности психологического пространства (СПП): p = 0,015.

У подростков, страдающих психосоматическим заболеванием, в отличие от здоровых, в большей степени нарушены общая суверенность психологического пространства личности и все ее частные показатели. Нарушен приемлемый для них режим телесных контактов, присутствует давление со стороны родителей в сфере ценностей и способов действия, родители стремятся контролировать социальные контакты подростков, имеют место неприятные переживания по поводу вторжений в личное пространство вещей подростка и т.д.

Далее мы сравнивали частоту встречаемости эмоциональной зависимости от матери и от отца в группах больных и здоровых подростков. У больных подростков достоверно чаще встречается эмоциональная зависимость от матери (р = 0,0045), но не от отца (р > 0,1). Чрезмерная эмоциональная привязанность к матери нехарактерна для здоровых подростков. Ряд авторов считает, что именно отношения с матерью являются определяющими в процессе психологического отделения. Данные результаты подтверждают ведущую роль матери и зависимости от нее в течение сепарации.

Сравнивая показатели когнитивной зависимости от матери и от отца в группах больных и здоровых подростков (р = 0,273, р = 0,374 соответственно) мы не обнаружили различий.

Поскольку сепарация от родителей — процесс очень сложный и не подлежащий замерам с помощью отдельного инструмента, мы считаем, что о качестве протекания данного процесса можно судить по тому, как взаимосвязаны ее компоненты. Это предположение привело нас к решению изучить роль эмоциональной зависимости от матери как базового проявления нарушений процесса сепарации. По итогам исследования эмоциональной зависимости от матери вся выборка (и больные и здоровые) была разделена на 2 группы, но уже по другому критерию — наличию/отсутствию эмоциональной зависимости от матери. Проведенные расчеты показали, что между группами подростков, имеющих эмоциональную зависимость и не имеющих таковой, обнаруживаются неслучайные различия по уровню семейной тревоги и суверенности психологического пространства.

Нарушения факторов семейной системы также констатируются достоверно чаще в семьях подростков с ПСР, чем в семьях здоровых подростков. Они отмечают у себя в семье негативный эмоциональный фон (р = 0,039), повышенную тревогу, неуверенность в себе и обостренное чувство вины. Такие подростки отмечают в своих семьях низкую степень семейной сплоченности (р = 0,04) — эмоциональной связи между членами семьи, дистанцированность их друг от друга. Больные подростки характеризуют свои семьи как не поощряющие к самоутверждению, независимости и самостоятельности в обдумывании проблем и принятии решений (р = 0,015), причем наибольшая депривация стремления к независимости характерна для подростков с язвенной болезнью желудка (р = 0,045). Воспитание в семье подростков с ПСР имеет более низкую ориентацию на достижения (р = 0,004). Низкая степень иерархичности семейной организации, ригидность семейных правил и структур также характерны для семей таких подростков (р = 0,018).

Изучив специфику факторов семейной системы у подростков, мы выяснили, что многие показатели функционирования семьи у подростков с ПСР значимо отличаются от таковых у здоровых подростков. В частности, в экспериментальной группе наблюдаются нарушение процесса сепарации, проявляющееся в более высокой эмоциональной зависимости от матери, нарушение психологических границ, причем по всем показателям суверенности психологического пространства, высокий уровень семейной тревоги, несформированная иерархия правил, более низкий уровень сплоченности и эмоциональных связей в семье. Однако мы хотели бы отметить, что наиболее очевидные различия наблюдаются именно в характеристиках семьи, описывающих центральную задачу системы — отделение подростка.

Следуя дальнейшей логике нашего исследования, мы отобрали группу здоровых подростков и группу подростков с заболеваниями желудочно-кишечного тракта, у которых наблюдаются описанные нами выше дисфункции семейной системы (28 и 23 подростка соответственно). Причем, отбирая группу здоровых подростков, мы также контролировали их успеваемость и адаптированность к школе посредством анализа оценок в журнале и бесед с учителями. Мы постарались учесть, что стабилизация дисфункциональной системы может происходить как за счет психосоматического заболевания подростка, так и за счет нарушений в его социальной адаптации. Таким образом, уравнивая контрольную и экспериментальную группы по уровню адаптированности и характеру дисфункций семейной системы, мы попытались вычленить именно личностные особенности подростка, которые позволяют ему совладать с нарушениями в семье без снижения социальной адаптированности или развития психосоматической симптоматики.

Результаты исследования показали, что у подростков из контрольной и экспериментальной групп статистически значимо различается уровень развития рефлексии. Результаты представлены в таблице № 1.

Используя дополнительные переменные, мы также обнаружили отрицательную корреляцию между уровнем рефлексивности и частотой простудных заболеваний. Независимо от наличия психосоматического заболевания, существует обратная корреляционная зависимость (p-level 0,05) между уровнем рефлексивности и частотой простудных заболеваний у подростков из семей с описанными выше дисфункциями. Таким образом, мы можем говорить о развитии рефлексии как достаточно значимого ресурса подростков в совладании со стрессом, который несет в себе дисфункциональная семья.

Различий в степени выраженности алекситимии среди групп больных и здоровых подростков обнаружено не было. Анализ копинг-стратегий у подростков контрольной и экспериментальной групп также не дал устойчивых различий в преобладании тех или иных копингов. Возможно, влияние дисфункциональных проявлений семьи для изучаемых нами личностных особенностей оказывается более сильным, нежели личностные возможности подростков в их развитии.

В заключение нашего исследования мы можем сделать следующие выводы:

1.

Существуют психологические особенности семейной системы у подростков с психосоматическими расстройствами. Эти особенности проявляются в нарушении процессов сепарации подростка в семье, таких как повышенная эмоциональная зависимость от матери и депривация психологического пространства, а также в высоком уровне семейной тревоги, несформированной иерархии правил, более низком уровне сплоченности и эмоциональных связей в семье.

2.

Среди описанных нами дисфункций семейной системы у подростков с психосоматическими расстройствами наиболее отчетливо проявляются нарушения, связанные с процессами отделения подростка.

3.

Установлено, что в семьях с указанными выше дисфункциями здоровые подростки и подростки с ПСР отличаются по уровню развития рефлексии. По-видимому, рефлексия подростка может сыграть важную роль в преодолении стресса, который несет дисфункциональная семья.

4.

Установленные взаимосвязи позволяют обосновать психотерапевтические стратегии помощи подростку с психосоматическим расстройством как на внутриличностном уровне, так и на уровне системных межличностных интервенций.

Abstract

The article represents the study of system dysfunctions in the families of adolescents aged from 11 to 14. The research has covered 77 adolescents, including 35 adolescents with psychosomatic gastrointestinal tract disorders. The goal of research is studying the specifics of personal peculiarities, which help an adolescent to cope with negative stress impact of dysfunctional family system.

Dysfunctions of family system and processes of an adolescent’s separation in the family have been studied with the following techniques: CTR (emotional component of identification), "studying of self-esteem by means of ranking procedure", which provides information about cognitive dependence from parents. "Family Anxiety Analysis" by E.G. Eidemiller and V. Justickis, "Sovereignty of Personal Psychological Space" questionnaire by S.K. Nartova-Bochaver, Family Surrounding Scale (FSS) for evaluation of social climate in families; Family Adaptation and Cohesion Scale (FACES-3). Theoretical analysis of adolescent personal characteristics, which reduce the risk of somatization, allowed to allocate reflection, alexithymia level and coping strategies character, which have been studied by means of the following techniques: R. Lazarus and S. Folkman coping test, "Reflexivity level" questionnaire by A.V. Karpov and Toronto Alexithymia Scale adapted at the V.M. Bekhterev Institute.

The obtained results show the impairment of separation processes of an adolescent with psychosomatic disorder in the family, increased emotional dependence on mother and deprivation of psychological space, high level of family anxiety, non-arranged hierarchy of rules, lower level of cohesion and emotional relations in a family. The role of an adolescent’s developing reflection as a defence factor in coping with stress impacts of dysfunctional family system in the conditions of psychosomatic disorder has been established.

The study was conducted with financial support of RHSF (Russian Humanitarian State Foundation) within the framework of scientific researches project “Quality of Early Mother-Child Contact in the Psychosomatic Factors System as a Condition of Health Keeping”, project № 14-06-00085.

 

Historically, the time limits of adolescent age, a period between junior school age and a person’s final psychological, social and material autonomy, keep expanding. Change of an adolescent’s position in the family, processes of separation as well as review and change of family limits are the main alterations of this period. The complexity of the above mentioned process, crisis in many spheres of modern society, environmental deterioration and other reasons, traditionally described in this case, increase the prevalence of somatic diseases, caused to a considerable degree by psychological factors. The prevalence of psychogenic somatic and somatogenic disorders in child and adolescent age is significant. Moreover, it is constantly increasing. Nowadays more and more chronic diseases "become younger".

Long-termed researches of child and adolescent susceptibility to diseases, role of psychological and social hazards in occurrence, provoking and development of clinical manifestations, course and outcome of psychosomatic disorders as well as personal peculiarities of such patients and their responses to a disease and life situations caused by diseases allowed to state the concept of psychosomatic disorders development mechanisms. Psychosomatic medicine is the subject of works of F. Aleksander, V. Brautigam, V.D. Mendelevich, B.D. Karvasarskiy, S.A. Kulakov, D.N. Isaev and others. The existing theories are integrated and psychological peculiarities interrelated with the occurrence of certain disease in different periods of life are considered.

A block of theories stating that social factors caused by family environment provide the basis for chronic stress and psychosomatic pathology occurrence occupies a special place. System approach in family psychology, which regards a disease as a dysfunctional stabilizer of family system, belongs to these theories. Conceptual ground for system approach to family is described in the works of system family psychotherapy representatives: М. Bowen, V. Satir, G. Bateson, E.G. Eidemiller, V. Justickis, A.V. Chernikov, A.Ya. Varga and others. The authors describe a normally functioning family as a family, whose members are responsible for different functions. Therefore the need for growth and changes of both family in general and each family member is satisfied.

Both theoretical justifications and experimental researches, which confirm the influence of a family on mental and somatic well-being of a child in it, are quite largely represented in modern psychology. However, we think that the potentials of a child’s personality and especially an adolescent’s personality (due to the developing separation and autonomy at this age period) in the process of coping with stress impact of a dysfunctional family in the conditions of psychosomatic disorder are understudied.

Thus, we need to study the specific system dysfunctions, existing in families with a teenager who suffers from psychosomatic disorder, and only then find out whether there are differences in personal peculiarities of healthy and sick adolescents, which live in families with the dysfunctions we have revealed earlier.

The goal of our research is studying the specifics of personal peculiarities, which help an adolescent to cope with negative stress impact of a dysfunctional family system.

The tasks of our research are:

1.

to arrange and conduct an empirical study, devoted to the peculiarities of family systems of adolescents, suffering from psychosomatic disorders (on the example of GI tract diseases);

2.

to allocate the criteria of family dysfunctions in adolescents with gastrointestinal tract diseases;

3.

to define what personal peculiarities enable adolescent to cope with negative stress impact of dysfunctional family system and will be connected with somatic well-being of an adolescent.

The questions of an adolescent’s separation and development of new functional psychological limits in the family among the main tasks for the family system to face in the period of an adolescent’s growing-up. We assume that the impairment of the process of psychological separation from parents and, as a consequence, dependency relationships will be some of the main components of family system impairments. Murrey Bowen wrote that the family "becomes unstable and disorganised, intrafamilial tension increases" when it collides with a child who is ready to separate [8]. On the other hand, a disease will effect the relationships and can also be associated with the impairment of this process as a real life event. In either case, some authors state that the issues of separation and existence of a psychosomatic disease will be interrelated [1; 8; 9].

Thus, in our opinion, the characteristics, which describe the process of emotional and cognitive separation of an adolescent from parents, preservation/deprivation of psychological limits, "analysis of family anxiety" as a more general index, which shows an unfavourable course of separation and triangulation processes, are the most significant indices of family system functionality at the regarded stage of family development. Basing on the theories of E.G. Eidemiller, A.Ya. Varga and others, we consider the impairments of family system factors as the impairments of family adaptation, family cohesion and family communication.

We used CTR (emotional component of identification) technique, "studying of self-esteem by means of ranking procedure", which provides information about cognitive dependence from parents; "Family Anxiety Analysis" by E.G. Eidemiller and V. Justickis; "Sovereignty of Personal Psychological Space" questionnaire by S.K. Nartova-Bochaver, Family Surrounding Scale (FSS) for evaluation of social climate in families; Family Adaptation and Cohesion Scale (FACES-3) to diagnose the described characteristics of family system.

Theoretical analysis of adolescent personal characteristics, which reduce the risk of somatization [2; 3; 5], allowed to allocate the following ones: reflection development, alexithymia level and coping strategies character. We used such techniques as R. Lazarus and S. Folkman coping test, "Reflexivity level" questionnaire by A.V. Karpov and Toronto Alexithymia Scale adapted at the V.M. Bekhterev Institute to implement this task of our study.

The research included 77 adolescents aged from 11 to 14, 35 of them we diagnosed with chronic gastroduodenitis and nonspecific ulcerative colitis (the adolescents’ diagnosis is confirmed in the department No. 2 of Kursk Regional Children’s Hospital, the adolescents were diagnosed according to anamnesis data, complaints, clinical indices of GI tract condition, functional methods of research) and 42 healthy adolescents with an average performance and school adaptation. The research was conducted on the base of children’s clinical hospital and Kursk schools.

Mann — Whitney U Test, Fisher angular transformation φ* and Spearman's rank correlation coefficient rs were used for statistical data processing.

Let us pass to the discussion of results.

Settling our first task, we compared psychological characteristics of family systems in sick and healthy adolescents. This comparison allowed us to obtain the following results.

We found out significant differences between the groups of sick and healthy adolescents according to all the indices of sovereignty of personal psychological space.

Physical body sovereignty (PBS): p = 0.024.

Territory sovereignty (TS): p = 0.043.

World of things sovereignty (WTS): p = 0.003.

Habits sovereignty (HS): p = 0.049.

Social relations sovereignty (RS): р = 0.0013.

Values Sovereignty (VS): р = 0.012.

Integral index of psychological space sovereignty (PSS): p = 0.015.

Adolescents with a psychosomatic disease are characterised by more severe impairments of general sovereignty of personal psychological space and all its private indices, unlike their healthy peers. The regime of body contacts acceptable for adolescents is impaired, they are exposed to the parental pressure in terms of values and modes of actions, the parents try to control the adolescents’ social contacts, the adolescents have unpleasant experiences related to the invasions in the personal space of their things etc.

Further, we have compared the incidence of emotional dependence from mother and father in the groups of sick and healthy adolescents. Sick adolescents significantly more frequently show emotional dependence from mother (р = 0.0045), though not from father (р > 0.1). Excessive emotional affection to mother is not typical for healthy adolescents. Some authors consider relationships with mother to be crucial in the process of psychological separation. These results confirm the key role of mother and dependence on her in the course of separation.

No differences have been found in the indices of cognitive dependence from mother and father in the groups of sick and healthy adolescents (р = 0.273 and р = 0.374 respectively).

Separation from parents is a very complicated process, which can not be measured by a single tool. Therefore we consider it possible to assess this process by studying the interrelation of its components. This assumption made us study the role of emotional dependence from mother as a basic manifestation of the impaired process of separation. According to the results of study devoted to emotional dependence from mother, the entire sample (both the sick and the healthy) was divided into 2 groups. The division was based on another criterion — presence/absence of emotional dependence from mother. Calculations have shown non-random differences according to the level of family anxiety and sovereignty of psychological space between groups of adolescents with or without emotional dependence.

Impairments of family system factors are also registered significantly more often in the families of adolescents with PSD than the in families with healthy adolescents. They discover increased anxiety, lack of self-confidence, strong sense of guilt and negative emotional background (р = 0.039) in their families. Such adolescents discover low level of family cohesion (р = 0.04) in their families, i.e. emotional relations between family members and their distancing from each other. Sick adolescents say that their families do not encourage them to be self-assertive and independent and to rely on themselves in considering problems and making decisions (р = 0.015). Moreover, adolescents with gastric ulcer are characterised by the strongest deprivation of their striving for independence (р = 0.045). Upbringing in the families of adolescents with PSD has a lower orientation towards achievements (р = 0.004). Low level of hierarchy in family organization, rigidity of family rules and structures are also typical for families of such adolescents (р = 0.018).

After studying the specifics of family system factors in adolescents, we have found out that many indices of family functioning in adolescents with PSD significantly differ from these in healthy adolescents. For example, the experimental group shows the impairment of separation process manifested in stronger emotional dependence from mother, violation of psychological limits according to all the indices of sovereignty of psychological space, high level of family anxiety, undeveloped hierarchy of rules, lower level of cohesion and emotional relations in the family. However, we would like to notice that it is the characteristics of the family, which describe adolescent’s separation (central task of the system), that represent the most evident differences.

Following the further logics of our research, we have selected a group of healthy adolescents and a group of adolescents with gastrointestinal tract diseases, who show the above mentioned dysfunctions of family systems (28 and 23 adolescents respectively). Besides, while selecting the group of healthy adolescents, we also controlled their performance and school adaptation through the analysis of school marks and interviews with teachers. We tried to take into account that the dysfunctional system can be stabilized due to both adolescent’s psychosomatic disease and impairments in his social adaptation. Thus, levelling the control and experimental groups according to the adaptation level and character of family system dysfunctions, we tried to single out an adolescent’s personal peculiarities, which enable him to cope with impairments in the family without reduction of social adaptation or development of psychosomatic symptoms.

Research results have shown that the level of reflection development in adolescents from control and experimental groups statistically differ. The results are represented in Table 1.

Using additional variables, we also discovered negative correlation between reflection level and cold-related diseases frequency. Irrespectively of the existence of psychosomatic disease, there is an inverse correlation dependence (p-level 0.05) between reflection level and cold-related diseases frequency in adolescents from families with the above mentioned dysfunctions. Thus, we can speak of the development of reflection as quite a significant adolescent’s source for coping with stress caused by a dysfunctional family.

No differences in alexithymia intensity among the groups of sick and healthy adolescents have been revealed. Analysis of coping strategies in adolescents of control and experimental groups has shown no stable differences in the dominance of various copings either. The influence of dysfunctional manifestations of a family for the studied personal peculiarities is possibly stronger then personal potentials of adolescents in their development.

In the end, we can draw the following conclusions:

There are psychological peculiarities of family system in adolescents with psychosomatic disorders. These peculiarities are manifested in the impairments of an adolescent’s separation processes in the family, increased emotional dependence on mother and deprivation of psychological space as well as in the high level of family anxiety, non-arranged hierarchy of rules and lower level of cohesion and emotional relations in the family.

2.

Among the described family system dysfunctions impairments associated with an adolescent’s separation processes are more clearly developed in adolescents with psychosomatic disorders.

3.

It is found that healthy adolescents and adolescents with PSD in families with the above mentioned dysfunctions differ according to the level of reflection development. An adolescent’s reflection could possibly play an important role in coping with stress caused by dysfunctional family.

4.

The established interrelations enable to justify psychotherapeutic strategies of assisting an adolescent with psychosomatic disorder at both intrapersonal level and system interpersonal interventions level.
