Психосоматогенез и эволюционный процесс

Аннотация

Статья посвящена описанию авторской гипотезы эволюционно обусловленной природы психосоматогенеза и является развитием ранее опубликованных тезисов. Гипотеза реализовала задачи: а) определение общего условия запуска психосоматогенеза, включающего в себя как частные случаи, те условия, которые были определены ранее проведенными исследованиями, и верное для человека и остального животного мира; б) определение общей функции психосоматогенеза и той, более общей системы, в которой эта функция является необходимой. Опираясь на некоторые психологические, медицинские и биологические концепции и современные данные физиологии, нейробиологии, молекулярной и эволюционной биологии, был выдвинут следующий ряд предположений. 1. Любые соматические болезни являются инструментом эволюционного процесса, подобно апоптозу и репликативному старению. 2. Психическое и поведенческое развитие особи, осуществляемое ею в индивидуальной жизни, накопление опыта решения тех задач, для которых у особи нет стереотипного или известного варианта действий, появление новых способностей и усложнение психической организации, а также стимулирование этим развития соответствующих физиологических механизмов является частью целостного эволюционного процесса. Причем здесь речь не идет об освоении нового поведения и развитии психики в процессе становления детской особи взрослой. 3. Функцией телесных болезней является способствование отбору особей, активно участвующих в эволюции форм и средств психического через уменьшение выживаемости особей, недостаточно активно участвующих в формировании новых поведенческих «решений» и форм психической регуляции, либо стимулирование участия особей в этом развитии. Главным условием запуска процесса порождения телесных болезней является ситуация, когда живое существо перестает участвовать в эволюции психического, когда его образ внешней и внутренней жизни препятствует развитию его же психической организации.

Делается вывод, что предложенная гипотеза не противопоставляет себя ранее накопленному объему представлений о закономерностях психосоматогенеза и является новой концепцией, которая может дать взгляд на несовпадающие психосоматические представления как частные случаи более общего, объединить в одном поле противоположные взгляды и создать контекст для дальнейших исследований. В рамках представленной концепции обсуждается вопрос об исследовании механизма психосоматогенеза и о взаимодействии механизмов психосоматогенеза и старения как инструментов эволюционного процесса. Приводятся конкретные предположения о возможном механизме психосоматогенеза.

Наверное, никто не будет спорить с тем фактом, что физическое и психическое в живом существе составляют единство. Также бесспорно, что обе эти стороны живого обладают достаточной автономностью и до определенной степени подчиняются автономным же законам функционирования. Отсутствие сколько-нибудь полного понимания того, как именно соотносятся телесное и психическое в живой материи, корреспондируется с отсутствием разрешенности более частных вопросов. Например, вопроса о том, как именно взаимодействуют телесное и психическое в порождении телесных болезней.

Развитие взглядов на этот вопрос можно уподобить витку диалектической спирали. Известно, что и в самом лечении и в понимании процесса порождения телесных болезней врачи античности и средневековья исходили из представлений о единстве душевного и телесного в человеке. К сожалению, отставание развития психологии от бурного развития естественных наук в эпоху возрождения и последующие столетия привело к тому, что к 70-м годам XIX века медицина в своем понимании причин болезней тела и разработке своих практических средств стала опираться исключительно на познанные закономерности функционирования телесного, физического, сознательно очищаясь от всего психологического. Именно в это же время начала оформляться как научная дисциплина материалистическая психология. И ее развитие и развитие медицинской психиатрии привело чуть позже к появлению современной психотерапии. И именно через одного из отцов психотерапии З. Фрейда понимание значения психологических причин телесных болезней стало постепенно возвращаться в науку и практику исцеления. Однако вначале главным образом как представление о конверсии истерии, как появление телесных симптомов без реальных медицинских причин [25; 26]. Следующей вехой в развитии психосоматических представлений стало описание механизма превращения определенных внутридушевных конфликтов в определенные же и, что важно, реальные болезни тела. Начало этого этапа более всего можно связать с работами Ф. Александера [32]. Болезни, для которых механизм психосоматогенеза был выявлен, и стали называться психосоматическими. Естественно, что ситуация, когда возможность относить соматическое заболевание к психосоматическим определяется тем, выявлены ли для него (уже!) механизмы превращения психологических причин (возможно, вкупе с физиологическими) в телесный недуг, ведет к принципиальной возможности расширения списка психосоматических болезней! Что, кстати, и можно было наблюдать после выхода в свет александеровской «Психосоматической медицины». Понятно, что такое положение привело к появлению представления о правомерности рассмотрения любого соматического заболевания как возможно имеющего существенные психологические причины, то есть как психосоматического. А также открыло дорогу появлению идеи о том, что психологические причины можно не только рассматривать на равных по отношению к чисто телесным в генезе соматического заболевания (естественно, рассматривая их во взаимодействии), но считать их более глубинными основаниями любого физиологического процесса порождения телесного заболевания.

Однако психосоматическая позиция в рассмотрении причин порождения телесных болезней на сегодняшний день все еще не является бесспорной в профессиональном сообществе. И споры идут отнюдь не только вокруг расширения списка психосоматических болезней. Несмотря на то что давно понятна связь центральной нервной системы и «событий», происходящих в ней, с «событиями» в вегетативной нервной, эндокринной и иммунной системах, для многих специалистов-медиков вопрос о возможной существенной роли психического в генезе хотя бы некоторых соматических болезней является более чем спорным. Необходимо признать, что такая ситуация вызвана не только сложностями диалога между различными парадигмами. Но также отсутствием исчерпывающих эмпирических доказательств психосоматической идеи и конечной проработанности теоретических моделей психосоматогенеза. Таким образом, развитие научных исследований природы и закономерностей психосоматических взаимосвязей остается вполне актуальным.

Исследование природы превращения психических, внутридушевных событий, закономерностей, процессов (в единстве с телесными явлениями или самих по себе) в патофизиологические процессы, в соматические болезни удобно разделить на три вопроса:

1.

Какие именно психические процессы и явления превращаются в телесную болезнь или способствуют ее генезу и каковы условия этого генеза?

2.

Каков механизм этого превращения?

3.

Какова функция, смысл этого превращения? И, соответственно, в какой системе, в каком контексте надо рассматривать эту функцию, этот смысл?

В огромном многообразии теоретических и эмпирических исследований в области психосоматики [1; 3; 4; 5; 6; 8; 9; 11; 12; 13; 14; 17; 18; 19; 20; 21; 22; 23; 24; 25; 26; 27; 28; 29; 30; 31; 32; 33; 34; 35; 36; 37; 39; 40; 41; 42; 43; 44; 45; 46; 47; 48; 49; 50; 51; 52] наибольшее внимание уделяется исследованию связи различных психических процессов и закономерностей, психических и личностных особенностей, а также семейных и социальных условий их возникновения с образованием телесных болезней [1], то есть первому вопросу. Некоторое внимание уделяется построению моделей механизма психосоматогенеза, однако на настоящий момент достаточно проработанной, на наш взгляд, можно считать только модель Ф. Александера. Смысл болезней, определяемых как психосоматические, их функции рассматриваются обычно в контексте жизненного функционирования человека или его семьи и понимаются или как нахождение компромиссного решения в конфликте мотивов или как источник и стимул к восстановлению психической целостности. Ранее нами была предложена более широкая концепция функции психосоматогенеза, в которой последний рассматривается как инструмент психического аспекта биологической эволюции [29].

В данной работе мы развиваем ранее опубликованную тезисную форму изложения наших предположений. В ней мы ставим целью не сделать полное теоретическое обоснование нашей гипотезы, а, скорее, описать контекст, в котором она была создана, указывая на факты и теоретические представления, реально поспособствовавшие ее кристаллизации. А также обозначить контекст, в котором наша гипотеза эволюционно-обусловленной природы психосоматогенеза может работать как теоретическая концепция.

Если обобщить результаты обширных исследований связи различных психологических процессов и явлений с порождением соматических заболеваний, который наработали, с одной стороны, специалисты в области исследования психосоматических связей в своих эмпирических исследованиях, с другой — психотерапевты на основе наблюдений в своей практике, выбрав при этом наиболее повторяющееся, то можно сформировать небольшой список психических условий, способствующих сохранению здоровья и, соответственно, при их неисполнении — порождению телесных болезней, который, по-видимому, можно считать достаточно достоверным.

1. Условием, способствующим сохранению здоровья (а зачастую и исцелению), является способность, а точнее, такая наша внутренняя организация, которая не препятствует знать о своих потребностях и удовлетворять их. Соответственно, если наше «внутридушевное» устройство таково, что мы вынуждены вытеснять некоторые части себя в бессознательное, не замечать некоторые свои потребности и чувства, если живем, опираясь на неизменяемые правила, а не руководствуемся реалиями текущего момента, то это способствует генезу соматического заболевания.

2. Предыдущее зависит от способности разрешать внутренние конфликты, способности, которая сама по себе также является условием, способствующим сохранению соматического здоровья. Причем телесный симптом, зачастую, рассматривается именно как патологический компромиссный способ разрешения конфликтов и восстановления целостности.

3. Условием, способствующим психосоматогенезу, является стресс. Точнее, долгий или частый стресс. Когда мы живем таким образом, что стресс или не долог или мы находим способ реализации стрессовой энергии, то стресс не способствует возникновению соматических заболеваний.

4. Также признаны способствующими формированию соматических заболеваний психотравмы, потери. Однако, по-видимому, их можно считать разновидностью стресса.

Еще одним условием сохранения соматического здоровья, по-видимому, можно считать возможность естественного вхождения в идентичность необходимых новых (и возвращение ранее вытесненных за пределы сознательного «Я») уже «созревших» качеств. Это условие является непосредственным следствием представлений, развиваемых в некоторых психотерапевтических методах. Так, автор «процессуальной работы» А. Минделл [43] рассматривает телесный симптом как остановленный, «замороженный» процесс развития идентичности, а механизмом его формирования может быть застревание, «топтание» этого процесса вокруг «края», барьера идентичности. И хотя данное условие, взятое само по себе, нельзя отнести к выше озвученному «общему» списку, а с другой стороны — его достаточно правомерно рассмотреть всего лишь как аспект способности к разрешению внутренних конфликтов, оно было настолько важно в создании контекста нашей гипотезы, что мы выделяем его как отдельное.

Заметим, эти пять условий можно считать относящимися, скорее, только к людям, обремененным культурой и цивилизацией, и связанными с ними невротическими видами развития и противоречиями, но не к животным. И если считать механизм психосоматогенеза универсальным в природе, то должно быть более общее условие, включающее предыдущие как частный случай и «работающее» и в животном мире!

Запуск психосоматогенеза также связывается с определенными эмоциями, состояниями и расположенностью к определенному виду активности. Одна из концепций, развиваемая в этом контексте, сыграла важнейшую роль в формировании нашей гипотезы и требует, чтобы мы также остановились на ней подробнее. В.С. Ротенберг и В.В. Аршавский разработали концепцию «поисковой активности» — активности, направленной на поиск способов изменения неудовлетворяющей ситуации в целом, при условии неопределенности в результатах этой активности [21], развитой формой которой является творческая деятельность. Причем потребность в поисковой активности рассматривалась ими как одна из фундаментальных биологических потребностей. Основываясь на собственных экспериментах, на модели психосоматогенеза, порождаемого «отказом от веры в будущее» Д. Энгеля и А. Шмале [36], экспериментальных и теоретических исследованиях, связанных с концепцией «выученной беспомощности» А. Селигмана [50] (несколько иначе интерпретируя результаты экспериментов Селигмана, чем он сам), В.С. Ротенберг и В.В. Аршавский показали, что поисковая активность является условием сохранения здоровья, устойчивости к болезнетворным и стрессовым факторам. По их мнению, не знак эмоции важен для запуска соматогенеза, а то, стимулирует ли она или останавливает поисковую активность, таким образом концептуально обобщив тему «эмоции и психосоматогенез». Также, по мнению авторов этой концепции, направленность или отказ от поиска важны для сохранения здоровья более, чем сила и продолжительность стресса.

Заметим, что многие эксперименты, на которых основываются теоретические концепции поисковой активности и выученной беспомощности, были проведены на животных, поэтому здесь мы можем говорить об общем для человека и животных условии запуска психосоматогенеза. Однако это условие не включает в себя четыре общих условия, а значит, задача определения более общего условия, включающего в себя и поисковую активность, и вышеописанные, остается вполне актуальной. Более того, остается актуальным вопрос нахождения общей функции психосоматогенеза, которая и должна определять общее условие его запуска.

Понятно, что если относиться к соматическим болезням, в том числе психосоматическим, исключительно как имеющим причину в прошлом, например как к «поломке», сбою функционирования систем организма или неудачной встрече с агрессивной средой, то последняя задача должна рассматриваться как надуманная. Однако здесь, на наш взгляд, может оказаться весьма поучительной история с восприятием процесса биологического старения, которое также когда-то рассматривалось как результат «износа» тканей и систем живого организма. (И это притом, что живой организм отличается от неживого в том числе и способностью к самовосстановлению!)

Как известно, первые одноклеточные не обладали способностью к старению. В этом смысле они были бессмертными, но очень легко гибли от внешних причин. Когда появились колонии одноклеточных, то появился механизм апоптоза (клеточного самоубийства), который «реализовывал интересы» сообщества клеток как бы «в ущерб интересам» отдельных организмов. Механизмы старения, и прежде всего репликативное старение, — один из инструментов биологической эволюции, обеспечивающий реализацию необходимого эволюционного механизма: доля особей с новым приспособительным признаком, возникшим в результате изменений в генном коде, растет в популяции с каждым новым поколением.

Вернемся к психосоматическим и вообще соматическим болезням. Если исходить из представления, что болезни могут обладать «первичной» и «вторичной выгодой», что болезни являются патологическим механизмом разрешения внутренних конфликтов и восстановления целостности (о чем, кстати, свидетельствует психотерапевтическая практика — при нахождении других, более экологичных способов обретения этой целостности, болезни зачастую приходят к своему исцелению даже без медицинской помощи), то мы должны предположить, что организм обладает способностью как минимум удерживать начавшуюся болезнь, как максимум порождать ее. В условно-рефлекторных моделях порождения психосоматических симптомов используется термин «патологический условный рефлекс» [30]. Однако понятно, что если есть определенная условная реакция, то значит, есть и соответствующая ей безусловная, то есть врожденная биологическая способность к порождению болезненных симптомов. Что тогда должно свидетельствовать о какой-то ее полезной функции для индивида, популяции или для еще более широко рассматриваемых биологических систем. Наличие эффекта «ноцебо» (отрицательного плацебо) так же должно свидетельствовать в пользу биологической «заложенности» механизма болезнеобразования. Еще один важнейший аргумент в пользу понимания болезней как биологически запрограммированного механизма дает онтогенетическая концепция возникновения болезней В.М. Дильмана [10], в которой обосновывается, что причиной возникновения основных «болезней старения» (атеросклероза, гипертонии, сахарного диабета второго типа, онкологии) является продолжение работы механизмов, обеспечивающих индивидуальное развитие организмов в его молодом периоде жизни. Как и вышеописанные аргументы, эти теоретические выводы из многолетних эндокринологических и онкологических исследований В.М. Дильмана стали одной из предпосылок (причем наиважнейшей!) формирования первого тезиса нашей гипотезы. Еще одной из важных предпосылок первого предположения нашей гипотезы стала метафора естественного отбора, бытующая как минимум в научно-популярной литературе, метафора «природы» как персонифицированной сущности, имеющей «целью» сохранять, беречь полезные для эволюции организмы и уничтожать, точнее уничтожать быстрее, ненужные [2].

Итак, первое предположение нашей гипотезы состоит в том, что любые соматические болезни являются инструментом эволюционного процесса, подобно апоптозу и репликативному старению. Но на что же направлен этот инструмент?

Чтобы сформулировать второе предположение нашей гипотезы, нам придется напомнить главные вехи процесса становления взглядов современной эволюционной биологии. Как известно, Ж.Б. Ламарк предполагал, что новые нужные, адаптивные признаки формируются прижизненно путем «тренировки органов» и затем посредством некоего неизвестного механизма наследуются следующими поколениями. Отметим две важные особенности теории Ламарка:

а)

эволюционные изменения происходят и закрепляются в индивидуальной жизни особи;

б)

направляющее давление среды осуществляется не отбором, а обстоятельствами его индивидуальной жизни. (По-видимому, необходимость нечто изменить животному нужно как-то обнаружить, «прочувствовать» в своей собственной жизнедеятельности.)

 

Следующий этап развития эволюционной теории связан с именем Ч. Дарвина. Вопреки бытующему представлению, в своей теории он отнюдь не отказался от тезиса о наследовании прижизненно сформированных признаков, а лишь от ламарковского «стремления к совершенству» и добавил в качестве направляющего природного фактора естественный отбор. А вот возникший в середине прошлого века неодарвинизм, или синтетическая теория эволюции (СТЭ), принял на вооружение известный тезис А. Вейсмана о невозможности наследования сформированных в жизни отдельного животного признаков. СТЭ основана на некоторых дарвиновских положениях, генетике и молекулярной биологии. Причем данные последних долгое время подтверждали, что наследственная информация может передаваться только в одном направлении: от генотипа к фенотипу, от ДНК через РНК к белкам [16]. Заметим, что в СТЭ генные изменения случайны, а единственным направляющим фактором служит естественный отбор. Эволюция в этой концепции происходит скорее между поколениями, чем внутри каждого индивидуального развития.

Конечно, процесс биологической эволюции — это не только процесс развития анатомического строения и физиологического функционирования живых организмов. Это также и процесс развития форм поведения и форм психической жизни. И, конечно же, и развитие нервной системы, обеспечивающей психическую деятельность и регуляцию поведения. В отношении эволюции психического, оставаясь в рамках СТЭ, можно думать только о том, что случайные генетические изменения дают возможность появиться особям с различными вариантами изменений в нервной системе и одновременно в поведении и психике, а естественный отбор позволяет в следующих поколениях увеличиваться в общей популяции доли особей с новыми более адаптивными поведенческими и психическими способностями и соответствующим нервным аппаратом. Что важно, на основе этих новых способностей могут появиться такие новые варианты действий, которые, в свою очередь, при наличии у этого вида развитой нервной системы могут передаваться в поколениях через обучение.

Второе предположение нашей гипотезы (в отличие от вышеописанного) состоит в том, что психическое развитие каждой особи является частью эволюционного процесса в целом, или, иными словами, психический аспект процесса эволюционного развития видов принципиально может проходить «через» индивидуальное развитие каждой особи. (Причем здесь под психическим развитием мы, естественно, понимаем не собственно онтогенез, то есть развитие взрослой особи из детской, хотя и в этом развитии тоже могут возникать «эволюционно значимые наработки».) Подразумеваемое нами развитие удобно рассматривать как имеющее два уровня.

1.

Накопление опыта нахождения решений тех задач, для которых у особи нет стереотипного или известного варианта действий (заметим, что в некоторых концепциях развития психики она и возникает как аппарат, обеспечивающий возможность нестереотипного реагирования [7]).

2.

Появление новых способностей и усложнение психической организации, а также стимулирование этим развития новых физиологических механизмов, их обеспечивающих.

Естественно, что здесь мы также имеем в виду, что как новые психические и поведенческие «решения», так и вновь появляющиеся способности к производству этих «решений», могут какимто образом закрепляться и на генетическом уровне. Что, кстати, важно для тех видов, уровень развития нервной системы и психической организации которых не может обеспечить передачу накопленных поведенческих «наработок» следующим поколениям через обучение. Понятно, что формирование новых нервных, психических и поведенческих изменений может происходить вкупе с реализующими новое поведение изменениями анатомическими и телесно-физиологическими в самом теле. Естественно, полагать, что для особей видов, стоящих на разных уровнях развития, «предметом» эволюционного процесса, проходящего «через» психическое эволюционное развитие особи, будут являться совершенно разноуровневые способы психической регуляции. Например, для одноклеточных организмов им может служить оперантное научение (на которое они, вопреки бытующему представлению, способны [15]), а человек может развиваться, например, в «тонкостях» душевной и интеллектуальной организации. Нам представляется важным отметить здесь и то обстоятельство, что направляющее давление среды понимается здесь не как реализующееся через отбор из уже сформированных в предыдущих поколениях механизмов, а как происходящее при самом формировании новых психических механизмов в индивидуальной жизни. (Что, как мы полагаем, может влиять на скорость происходящего развития.)

Понятно, что такое понимание индивидуального психического развития как части общего процесса эволюционного развития возможно при условии «использования» эволюцией не только механизмов, описанных СТЭ, но также ламарковских механизмов формирования полезных признаков в индивидуальной жизни и их последующего наследования. И основания для такого смелого «оптимизма» в этом вопросе дают совершенные за последние десятилетия открытия современной эволюционной биологии, которые, чтобы не вдаваться в тонкости последней, мы просто перечислим:

а)

обратное кодирование белок — РНК — ДНК;

б)

эпигенетическое наследование;

в)

управление геномом мобильными генетическим элементами (МГЭ);

г)

управление организмом мутациями в нем.

Все перечисленное, по мнению многих современных специалистов в эволюционной биологии, указывает не только на теоретическую возможность наследования живыми существами приобретенных в индивидуальной жизни новых признаков, но также и то, что хоть сколько-нибудь полное понимание закономерностей и механизмов эволюционного процесса еще далеко впереди [16].

Третье предположение нашей гипотезы непосредственно определяет функцию и условие запуска психосоматогенеза. В нем мы предполагаем, что функцией телесных болезней является способствование отбору особей, активно участвующих в эволюции форм и средств психического через уменьшение выживаемости особей, недостаточно активно участвующих в формировании новых поведенческих «решений» и форм психической регуляции. Хотя также может оказаться, что особи, поставленные в условия болезни, могут становиться более расположенными к развитию, например через вынужденное болезнью изменение способа жизнедеятельности, и тогда нужно будет говорить уже о двух ролях болезней в этом способствовании психическому аспекту биологической эволюции. Соответственно, телесные болезни «включаются», когда живое существо перестает участвовать в эволюции психического, когда его образ внешней и внутренней жизни препятствует развитию его же психической организации. Содержание и сама необходимость этого эволюционного развития, «проходящего через» индивидуальную жизнь особи, может определяться:

а)

внешними задачами приспособления к окружающей среде, возникающей в его индивидуальной жизни;

б)

внутренними проблемами и уже возникшими внутри индивидуальными тенденциями развития (понятно, что о внутренних проблемах психической жизни животных, находящихся на примитивной стадии психического развития, мы сейчас очень мало можем судить);

в)

возможно, и тенденциями, заложенными в эволюционном процессе в целом. (В последнее время эволюционным биологам становится понятно, что сам эволюционный процесс развивается в своих формах и его можно и нужно рассматривать как единую систему [Там же].)

Очень важно заметить, что выдвинутая нами гипотеза не противопоставляет себя ранее накопленному объему представлений о природе психосоматогенеза. Более того, связывая генез всех соматических болезней с психическим аспектом глобального процесса эволюционного развития, рассматривая этот генез как его инструмент, эта гипотеза концептуально может объединить несовпадающие психосоматические представления как частные случаи более общего и даже объединить в одном контексте противоположные взгляды.

Прежде всего, возвращаясь к ранее приведенному списку условий запуска процесса психосоматогенеза, мы с очевидностью обнаружим, что

 

слышание себя и понимание своих потребностей, нахождение способов их удовлетворения,

 

нахождение путей разрешения внутренних противоречий и конфликтов,

 

позволение расширять собственную идентичность новыми, уже возникшими, необходимыми качествами и психическими (душевными) частями и нахождение способов это сделать,

 

нахождение «экологичного» способа реализации стрессовой и психотравматической энергии,

 

и, наконец, проявление поисковой активности и как инструмента трансформации нежелательной ситуации, и, вообще, как способа осуществления ориентировки в своем жизненном пространстве,

— в соответствующих контекстах и для соответствующих уровней развития психики являются необходимыми условиями участия особи в эволюции психического. (Понятно, что при более углубленном осмыслении закономерностей функционирования психики организмов, стоящих на низших уровнях развития, этот список будет расширен.) Таким образом, уже известные условия генеза психосоматических заболеваний становятся частными случаями более общего условия.

Также эта гипотеза, так широко трактующая психическую обусловленность генеза телесных болезней, не только снимает вопрос о том, какие болезни можно относить к психосоматическим. Она, с одной стороны, никак не опирается на какую-либо из конкретных позиций в отношении психосоматики, с другой — с ней могут согласиться не только адепты психосоматической парадигмы, считающие, что у всех или многих соматических болезней есть психологические причины (стоящие до соматических или имеющие значение наравне с ними), но также и адепты противоположных взглядов, считающие, что психические причины не могут существенно повлиять на процесс порождения телесных болезней. Что, естественно, создает дополнительный контекст для диалога между этими парадигмами.

Продолжая список полезных последствий принятия такого представления о природе психосоматики, мы можем отнести к их числу и возможность формулирования общей рекомендации по сохранению соматического здоровья. Мы здоровы настолько, насколько наш образ жизни позволяет нам реализовывать свой эволюционный процесс, и значит, нам необходимо пожизненно решать две задачи: а) понимать, «что хочет мой эволюционный процесс»; б) находить способ реализации вектора этого процесса в своей жизни. Из этого следует понимание, что не любое наше развитие и не любой поиск будет способствовать сохранению здоровья! И наше представление о необходимых векторах нашего развития может не совпадать с «представлением природы» на сей счет. (Необходимое условие, как известно из логики, не всегда является достаточным!)

Описанный выше ряд предположений об эволюционно обусловленной функции и условии запуска психосоматогенеза, который здесь совпадает с генезом соматических болезней вообще, как полагает автор, представляет собой концепцию, которая позволяет по-другому взглянуть на известное и создает контексты как для исследования еще неизвестного, так и объединения усилий специалистов, очень по-разному смотрящих на психосоматическую проблему. Естественно, эта концепция работает в контексте изучения природы соматических болезней всех представителей животного мира.

В рамках нашей концепции вслед за прояснением вопросов о функции психосоматогенеза и о главном условии его запуска также появляется возможность по-новому взглянуть и на ориентиры в исследовании самого его механизма. Автор должен признаться, что его представления об общем механизме (психо)соматогенеза еще совсем не достаточно проработаны, но тем не менее он считает уместным здесь ими поделиться. В данном контексте вопрос о механизме психосоматогенеза логично разделить на два: «Что именно в живом организме «определяет», что особь участвует или не участвует в эволюционном развитии психического?» и «Как (после этого) включается сам механизм формирования болезней?».

Исследование второго вопроса представляется нам делом, более относящимся к возможностям сегодняшнего дня, чем исследование первого, поэтому с него и начнем. Нам представляется совершенно логичным предположить, что физиологический механизм, реализующий «интересы» эволюционного процесса через формирование болезней, надо искать в чем-то связанном с работой генного аппарата. В настоящее время вряд ли кто-нибудь сможет усомниться в важности генной экспрессии в функционировании организма в целом. Современные нейробиологические исследования показывают зависимость экспрессии генов от психического состояния и одновременно влияние механизма генной экспрессии на ход процессов изменений в мозге и психике [38; 47; 48]. Наше предположение состоит в том, что исполняющим механизмом, включающим (по определенному сигналу) «режимы» здоровья и формирования болезни является активация или дезактивация определенной группы генов в соматических клетках. Эти «паттерн здоровья» и «паттерн болезни» определяют как организм в целом, и его различные органы будут реагировать на стресс, на инфекцию и т.д., включать и выключать различные физиологические системы, например регенерационную, иммунную, запускать различные механизмы, в конечном счете поддерживающие здоровье или запускающие болезнь либо просто способствующие болезни.

Но какая система определяет целесообразность включения одного из этих паттернов? Где ее стоит искать? Из самых общих соображений можно предположить, что, во-первых, со временем в каких-нибудь мозговых структурах высших животных обнаружатся нейроны, реагирующие на то, что особь производит поведенческое или психическое действие, которое ранее никогда не производила (конечно, отделяя то, что происходит вследствие обучения в ходе становления детской особи взрослой), и даже что это действие является «эволюционно значимым». По аналогии с «нейронами новизны» и «нейронами тождества» воспринимаемого сигнала. Во-вторых, исполняющий механизм болезни/здоровья может реагировать на начавшийся процесс генной записи в половые клетки. В-третьих, совершенно естественно предположить, что описанную выше активацию генных паттернов запускают определенные белки.

Не стоит говорить о возможностях здоровьесбережения, которые появятся при реальном обнаружении хотя бы одного из этих физиологических процессов и нахождении путей их искусственной активации или имитации! Однако совершенно ясно и то, что в ситуации несовпадения «целей» системы и ее элемента у первой может обнаружиться гораздо более широкий диапазон средств для предотвращения блокировки элементом действия инструментов, реализующих ее «цели»! Нахождение способов целенаправленно участвовать в эволюционном процессе психического, на наш взгляд, есть более гарантированный путь к управлению здоровьем!

Необходимо заметить, что каков бы ни был искомый общий исполнительный механизм психосоматогенеза, он в любом случае является механизмом механизмов, и уже открытые на сегодняшний день конкретные механизмы должны быть его частями или частными случаями. Нам представляется также, что «цели» различных инструментов эволюционного процесса должны быть согласованы, а конкретные механизмы взаимодействовать между собой. В нашем контексте это касается прежде всего взаимодействия механизмов психосоматогенеза и старения, примером которого могут послужить ранее предложенные нами две связанные гипотезы о природе онкологии. Опираясь на тот факт, что канцерогенез представляет собой рост количества неподвластных процессу старения клеток, несдерживаемый (в определенный ключевой момент) «подыгрывающим» иммунитетом, в первой гипотезе мы предположили, что этот процесс является «бунтом» против процесса старения, попыткой заместить стареющие ткани нестареющими [27; 28; 39]. «Бунт» против одного из важнейших инструментов эволюции, реализуемый на уровне особи, но являющийся глобальной тенденцией в эволюционном процессе. Однако, как мы знаем, «бунтовщики» умирают раньше. Эволюция «нашла способ» превращения попытки противостоять старению в смертельную болезнь. Во второй гипотезе мы предположили, что тот физиологический механизм, на который опирается этот «бунт», является механизмом регенерации ранних многоклеточных организмов, включавшийся при их травмировании (сходный с механизмом работы стволовых клеток), который теперь работает некорректно [28]. Причем теперь этот механизм включается чаще именно при психотравмировании, которое играет огромную роль у высокоразвитых организмов. (Эта гипотеза может быть совершенно самостоятельным от первого предположением.) Но отсутствие эволюционирования этого механизма, как нам представляется, также может объясняться «интересами» эволюционного процесса. Приведенные примеры, пытаясь пролить свет на природу онкологии, по-видимому, также указывают и на необходимость дальнейшей проработки вопроса о взаимодействии различных эволюционных инструментов и отношениях частных механизмов соматогенеза и общего.

Автор отдает себе отчет в том, что представленная концепция пока находится на начальной стадии разработки. Многое в ней требует более четкого обоснования, опоры на эмпирические данные и разрешения пока оставленных неопределенностей. Можно сказать,что она пока больше ставит вопросов, чем дает ответов. Однако, как мы знаем, удачная постановка вопросов во многом определяет успешность научного исследования и, возможно, именно в постановке задач дальнейших исследований природы психосоматогенеза и средств сохранения здоровья и окажется ее главное значение. Также, по нашему оптимистичному предположению, возможные возражения против изложенной здесь позиции приведут к дискуссии, более проясняющей исследуемый вопрос в целом.

Abstract

The paper describes the author’s hypothesis of the evolutionally determined nature of psychosomatogenesis and develops earlier published premises. The hypothesis has allowed us to achieve the following objectives: a) to find a general condition for activating psychosomatogenesis including the conditions found in previous research as special cases, which is true for a human and the rest animal world; b) to find a general function of psychosomatogenesis and another, more general system, in which this function is essential. Basing on some psychological, medical and biological concepts and current data of physiology, neurobiology, molecular biology and evolutionary biology, we have made some assumptions. 1) Any somatic disease is the instrument of an evolutionary process, like apoptosis and replicative ageing. 2) Individual's mental and behavioral development throughout lifetime, the accumulation of experience in solving the tasks, for which an individual has no stereotyped or known variant of actions, the emergence of new abilities and the sophistication of the mental structure, and, therefore, the stimulation of the development of relevant physiological mechanisms is a part of an integral evolutionary process. To that end, it is not a matter of mastering new behavior and the development of the psyche during the establishment of an adult individual from a child individual. 3) The function of body diseases is to assist in selecting individuals who participate actively in the evolution of the forms and means of the psyche by diminishing the survival of individuals who are not active in developing new behavioral "solutions" and forms of mental regulation or by encouraging individuals to participate in this development. The basic condition for activating the process of body diseases generation is a situation when a living creature stops participating in the evolution of the psyche, when his image of an external or internal life prevents from the development of his own mental structure.

We conclude that our hypothesis does not contradict to earlier accumulated volume of representations of the regularities of psychosomatogenesis. We assume that this is a new concept, which can give a view of different psychosomatic representations as special cases of a more general phenomenon, unite opposite views in one field and create a context for further research. We discuss the research into the mechanisms of psychosomatogenesis and the cooperation between the mechanisms of psychosomatogenesis and ageing as instruments of an evolutionary process within a framework of our concept. We give specific assumptions of the possible mechanism of psychosomatogenesis.

Probably, everyone agrees that body and psyche constitutes a whole in a living creature. Besides, both these parts of a living creature are obviously quite autonomous and are largely regulated by the autonomous laws of functioning. The lack of more or less complete understanding of the correlation between body and psyche in a living substance is accompanied by the lack of solutions for more special cases including the cooperation between body and psyche in the generation of body diseases.

The development of views of this issue can be compared to a wind of a dialectic spiral. It is known that ancient and medieval doctors relied on the representations of the unity of mind and body in a human in the treatment and understanding of the process of somatic diseases generation. Unfortunately, due to the retardation of psychology from rapidly developing natural sciences in the Renaissance and next centuries, medicine began to rely solely on the cognized regularities in the functioning of body and to purify itself consciously from anything psychological in its understanding of the disease of body and elaboration of its practical means by the 1870s. It is at that time that materialistic psychology began to establish itself as a scientific discipline. Its development as well as the development of medical psychiatry led to the emergence of modern psychotherapy a little later. It is through S. Freud, one of the fathers of psychotherapy, that the understanding of the meaning of psychological reasons of body diseases began to return gradually to the science and practice of healing. However, at the beginning, it primarily referred to the representation of the conversion of hysteria as the emergence of body symptoms without real medical reasons [25; 26]. The description of the mechanism of transforming certain intrapsychic conflicts into certain and, importantly, real diseases of body became the next stage in the development of psychosomatic representations. The beginning of this stage is primarily associated with the works of F. Alexander [32]. The diseases with a revealed mechanism of psychosomatogenesis were called psychosomatic. Naturally, if a possibility to refer a somatic disease to psychosomatic diseases depends on whether the mechanisms of transforming psychological reasons (possibly, together with physiological reasons) are (already!) revealed for it, we can enlarge the list of psychosomatic diseases! By the way, this is confirmed by the publication of Alexander’s Psychosomatic Medicine. Clearly, this situation led to the representation that we may treat any somatic disease as a disease with possible essential psychological reasons, i.e. as a psychosomatic disease. Besides, it gave way to the idea that physiological reasons can be treated as equal to purely physical reasons in the genesis of a somatic disease (naturally, if we consider them in their cooperation) and deeper bases for any physiological process of body disease generation.

However, a psychosomatic position in considering the reasons of body diseases is still disputable in a professional community now. The disputes are not only about the enlargement of the list of psychosomatic diseases. The relations between the central neural system and "incidents" in it and "incidents" in the vegetative neural, endocrine and immune systems are quite clear; however, the issue of the possible essential role of the psyche in the genesis of at least some of somatic diseases is highly disputable for many medical specialists. We should admit that this situation is caused not only by the difficulties of a dialogue between different paradigms. Another reason is the lack of exhaustive empirical evidence of a psychosomatic idea and finalization of the theoretical models of psychosomatogenesis. Therefore, the development of research into the nature and regularities of psychosomatic correlations remains quite relevant.

It is convenient to divide the research into the nature of transforming mental events and regularities (together with body phenomena or themselves) into pathophysiological processes in somatic diseases into three questions:

1)

What kind of mental processes and phenomena are transformed into a body disease or contribute to its genesis and what are the conditions of this genesis?

2)

What is the mechanism of this transformation?

3)

What is the function or meaning of this transformation? And, therefore, in what system or in what context should we treat this function or this meaning?

In the vast variety of theoretical and empirical research into psychosomatics [1; 3; 4; 5; 6; 8; 9; 11; 12; 13; 14; 17; 18; 19; 20; 21; 22; 23; 24; 25; 26; 27; 28; 29; 30; 31; 32; 33; 34; 35; 36; 37; 39; 40; 41; 42; 43; 44; 45; 46; 47; 48; 49; 50; 51; 52], the greatest attention is given to the research into the relations between mental process and regularities, mental and personal peculiarities as well as family and social conditions of their emergence and the formation of body diseases [1], i.e. to the first question. Some attention is given to creating the models for the mechanism of psychosomatogenesis; however, we assume that F. Alexander’s model is the most elaborated at the present moment. The meaning of diseases defined as psychosomatic and their functions are usually considered in the context of life functioning of a person and his family and are understood either as finding a compromise in the conflict of motives or as a source of restoring mental integrity and a stimulus to it. Earlier, we have proposed a larger concept of psychosomatogenesis functions, where the latter is treated as an instrument of a mental aspect of biological evolution [29].

In this work, we develop earlier published premises related to our assumptions. Here, we do not intend to give a full theoretical justification of our hypothesis. We rather want to describe a context, in which it was created, and to indicate the facts and theoretical representations that really enable to crystallize it. Besides, we aim at defining a context where our hypothesis of evolutionally determined nature of psychosomatogenezis might work as a theoretical concept.

If we generalize the results of a large research into the relations between various psychological processes and phenomena and the generation of somatic diseases obtained by specialists engaged in the study of psychosomatic relations, on the one hand, and psychotherapists who use observations in their practice, on the other hand and choose the most frequent ones, we will be able to create a small list of mental conditions that preserve health, and, respectively, produce body diseases in case they are not fulfilled, which can probably be considered quite valid.

1) Ability, or, to be more exact, our internal structure that does not prevent us from being aware of our needs and satisfying them is a condition that enables us to preserve health (and often to cure ourselves). Thus, if our mental structure makes us repress some parts of ourselves into the unconscious and ignore some of our needs and feelings, if we rely on unbreakable rules instead on the current reality, it contributes to the genesis of a somatic disease.

2) The previous condition depends on the ability to resolve internal conflicts, which itself is a condition that contributes to the preservation of somatic health. Moreover, a body symptom is often treated as a pathological compromise way to resolve conflicts and to restore integrity.

3) Stress is a condition that contributes to psychosomatogenesis (to be more exact, long or frequent stress). If stress is short or if we find a way to discharge stressful energy, stress does not lead to somatic diseases.

4) Psychological traumas and losses are also acknowledged as the conditions of somatic diseases formation. However, we can possibly treat them as a variant of stress.

Probably, another condition of preserving somatic health is the possibility of natural involvement of new already "mature" qualities (and the return of qualities that were earlier repressed outside the conscious Ego) in the identity. This condition is a direct consequence of representations developed in some psychotherapeutic methods. Thus, A. Mindell [43], the author of a "processual work", treats a body symptom as a stopped, "frozen" process of identity development; seizing or "trampling" of this process around the "edge" or barrier of identity can be a mechanism of its development. This condition itself cannot be referred to the above "general" list. Besides, it is quite reasonable to treat it only as an aspect of the ability to resolve internal conflicts. However, it was so important in creating the context of our hypothesis that we identify it as a separate condition.

Note that these five conditions can rather be referred only to people burdened by culture and civilization and related neurotic types of development and contradictions than to animals. Moreover, it we treat a mechanism of psychosomatogenesis as universal in nature, there must be a general condition that incorporates the previous ones as an specific case and "works" in the animal world as well!

The activation of psychosomatogenesis is also associated with certain emotions, states and disposition to a certain activity. One of the concepts developed in this context played a crucial role in the development of our hypothesis. Let us dwell on it. V.S. Rotenberg and V.V. Arshavsky elaborated the concept of "search activity" — activity aimed at searching for the ways to change a dissatisfying situation in general in case of uncertainty in the results of this activity [21]. Creative activity is a developed form of this activity. Moreover, they treated a need for search activity as one of the fundamental biological needs. Basing on their own experiments, the model of psychosomatogenesis caused by D. Engel’s and A. Shmale’s "refusal from a belief in the future" [36], and А. Seligman’s experimental and theoretical research into the concept of "learned helplessness" [50] (though interpreting Seligman’s results in a different way), V.S. Rotenberg and V.V. Arshavsky have shown that search activity is a condition of health preservation and resilience to malignant and stressful factors. They assume that the character of an emotion is not important for the activation of somatogenesis. It is important whether it stimulates or stops search activity. Therefore, they generalized the topic "emotions and psychosomatogenesis" in a conceptual way. Besides, the authors of this concept assume that focus on search or refusal from search is more important for health preservation than stress or the duration of stress.

Note that many experiments that underlie theoretical concepts of search activity and learned helplessness were conducted on animals; therefore, we can discuss here the condition of psychosomatogenesis activation that is common for a human and animals. However, this condition does not involve four general conditions, and therefore the objective of defining more general condition that would encompass search activity and the above conditions remain quite relevant. Moreover, the issue of defining a general function of psychosomatogenesis to determine a general condition of its activation remains relevant too.

It is clear that if we treat somatic diseases including psychosomatic diseases solely as the diseases caused by past events, as a "breakdown" or a failure in the functioning of the systems of organisms or as an unsuccessful encounter with an aggressive environment, the latter objective should be treated as artificial. However, we assume that a story with the perception of the process of biological ageing, which was also treated once as the result of the "ware" of tissues and systems of a living organism, can be quite didactic. (This is despite the fact that a living organism differs from a non-living organism, among other things, by its ability to self-restoration!)

It is well known that the first unicellular organisms were incapable of ageing. In this meaning, they were immortal; however, they died easily due to external reasons. When the colonies of unicellular organisms emerge, there occurred a mechanism of apoptosis (cell suicide), which seemed to "pursue the interests" of a community of cells by "violating the interests" of particular organisms. The mechanisms of ageing including first of all replicative ageing is one of the instruments of biological evolution, which provides the fulfillment of a necessary evolutionary mechanism: the share of individuals with a new adjusting sign acquired due to changes in a genetic code grows in a population with each new generation.

Let us return to psychosomatic diseases and somatic diseases in general. If we base on the idea that diseases may have a "primary benefit" and a "secondary benefit" and represent a pathological mechanism of solving internal conflicts and restoring the integrity (this is confirmed by psychoterapeutic practice — if there are other, more ecological ways of acquiring this integrity, diseases are often healed even without medical aid), we should assume that an organism is capable of at least holding a disease and as maximum causing it. Conditioned reflex models of psychosomatic symptoms generation use the term "pathological conditioned reflex" [30]. However, it is clear that if we have a certain conditioned response, it means that there is an appropriate unconditioned, i.e. congenital biological ability to produce malignant symptoms. In this case, it should be indicative of its useful function for an individual, population or even larger biological systems. The effect of "nocebo" (negative placebo) also proves that the mechanism of disease formation is biologically determined. Another crucial argument for treating diseases as a biologically programmed mechanism is given by the ontogenetic concept of disease occurrence of V.M. Dilman [10]. The concept proves that basic "ageing diseases" (atherosclerosis, hypertension, diabetes mellitus type 2, oncology) are caused by a continuing work of mechanisms that provide the individual development of an organism in the period of youth. Like the above arguments, these theoretical conclusions from V.M. Dilman’s long endocrinologic and oncological research became one of the prerequisites (of great importance!) for the development of the first premise of our hypothesis. Another crucial prerequisite of the first assumption of our hypothesis is a metaphor of natural selection, a metaphor of nature as a personified thing that protects organisms useful for evolution and destroys (as quickly as possible) useless organisms [2]. This metaphor is typical for nonfiction (and other literature as well).

Thus, the first assumption of our hypothesis is that any somatic disease is the instrument of an evolutionary process, like apoptosis and replicative ageing. What is purpose of this instrument then?

To formulate the second assumption of our hypothesis, we have to recall the main stages in the establishment of modern evolutionary biology. It is well known that J.B. Lamarck assumed that new necessary adaptive signs are developed throughout lifetime by "training of organs" and are inherited then by next generations by means of some unknown mechanism. Note two important peculiarities of Lamarck’s theory:

a)

evolutionary changes take place in individual’s life and are fixed in it;

b)

directive pressure of the environment is caused by the circumstances of individual life instead of selection. (Probably, an animal has to reveal or to feel a necessity to change something in its own activities.)

 

The next stage in the development of the evolutionary theory is associated with the name of Ch. Darwin. Contrary to a popular opinion, he was far from rejecting the affirmation that signs developed throughout lifetime are inherited. He refused only from Lamarck’s "pursuit of perfection" and added natural selection as a directive natural factor. In turn, Neo-Darwinism or modern synthesis emerged in the middle of the 20th century adopted a famous idea of A. Weisman that it is impossible to inherit signs developed throughout the lifetime of a particular animal. Modern synthesis is based on some Darwin’s provisions, genetics and molecular biology. Moreover, their data have long confirmed that hereditary information can be transferred only in one direction: from genotype to phenotype, from DNA through RNA to proteins [16]. Note that in modern synthesis, genetic changes are random and natural selection is the only directive factor. In this concept, evolution takes place rather between generations than inside every individual development.

Certainly, a process of biological evolution is not only a process of development of the anatomic structure and physiological functioning of life organisms. This is also a process of the development of forms of behavior and mental life. Besides, this is the development of a neural system that provides mental activity and regulations of behavior. With regard to the evolution of the psyche, modern synthesis enables us to assume that random genetic changes enable individuals with different variants of changes in the neural system, behavior and psyche to emerge, while natural selection enables to increase the share of individuals with new more adaptive behavioral and mental peculiarities and appropriate neural apparatus in a general population in the next generations. It is important that these new abilities may underlie new variants of actions, which, in turn, can be transferred from generation to generation if this species has a developed neural system.

The second assumption of our hypothesis (unlike the above) is that mental development of every individual is a part of an evolutionary process in general, or, in other words, a mental aspect of the process of evolutionary development of species can run through the development of every individual. (Here, mental development certainly does not mean ontogenesis, i.e. the development of an adult individual from a child individual; however, "practices significant for evolution" may also occur in this development".) It is convenient to reveal two levels in this development.

1.

The accumulation of the experience of solving the tasks for which an individual has no stereotyped or famous variant of actions (note that in some concepts of psyche development, it does emerge as an apparatus that provide non-stereotyped responding [7]).

2.

The emergence of new abilities and complication of mental structure, and, therefore, the stimulation of the development of new physiological mechanisms to provide them.

Naturally, we bear in mind here that new mental and behavioral "solutions" and new abilities for producing these "solutions" may somehow be fixed at the genetic level too. This is important for the species with such a level of development of neural system and mental structure that cannot provide the transmission of accumulated behavioral "practices" to the next generations through learning. It is clear that new neural, mental and behavioral changes may develop together with anatomic and physiological changes in the body itself responsible for new behavior. Naturally, species that are at a different level of development will have quite different ways of mental regulation as a "subject" of an evolutionary process that runs through the mental development of an individual. Thus, for unicellular organisms, it may be operant learning (despite a popular opinion, they are capable of this learning [15]), while a human may develop his mental and intellectual structure. It is important to note that the directive pressure of environment is understood here not as realized by selection from mechanisms that are already developed in previous generations, but as happening during the development of new mental mechanisms in an individual life. (We assume that it can influence the speed of development).

It is clear that this treatment of individual mental development as a part of a general process of the evolutionary development is only possible if evolution "uses" the Lamarck’s mechanisms of development of useful signs in the individual life and their further inheritance together with the mechanisms described in modern synthesis. Such bold "optimism" in this issue is supported by the latest discoveries in modern evolutionary biology. Here are a few of them:

a)

protein — RNA — DNA reverse coding;

b)

epigenetic inheritance;

c)

genome management by mobile genetic elements (MGE);

d)

mutations management by an organism. Many modern specialists in evolutionary biology assume that all the above is not only indicative of a theoretical ability of living creatures to inherit new signs acquired in an individual life. It also means that any complete understanding of the regularities and mechanisms of an evolutionary process in far ahead [16].

The third assumption of our hypothesis directly defines the function and condition of psychosomatogenesis activation. We assume here that the function of body diseases is to assist in selecting individuals who participate actively in the evolution of the forms and means of the psyche by diminishing the survival of individuals who are not active in developing new behavioral "solutions" and forms of mental regulation. However, individuals placed in the conditions of a disease may become more disposed to development, for example, through the changes in the way of activity caused by a disease. Then, we will have to discuss two roles of diseases in this contribution to a mental aspect of a biological evolution. Therefore, body diseases "occur" when a living creature stops participating in the evolution of the psyche, when his image of external or internal life prevents from the development of his own structure of the psyche. The content and the very necessity of the individual development that "goes through" the individual’s life may be defined by:

a)

external objectives of adjustment to the environment that occurs in individual life;

b)

internal problems and individual tendencies of development that have already occurred inside (it is clear that we know little about the internal problems of the mental life of animals at the primitive stage of mental development now);

c)

tendencies that underlie an evolutionary process in general (possibly). (Recently, evolutionary biologists have become aware of the fact that an evolutionary process develops in its forms and, therefore, should be treated as a uniform system [Ibid.]).

It is important to note that our hypothesis does not oppose earlier accumulated volume of representations of the nature of somatogenesis. Moreover, the hypothesis unites the genesis of all somatic diseases with a mental aspect of the global process of evolutionary development and treats this genesis as an instrument; therefore, it can conceptually unite different psychosomatic representations as special cases of a more general phenomenon and even unite opposite views in one context.

First of all, if we go back to the above list of conditions that activate the process of psychosomatogenesis, we will obviously find that

listening to ourselves and understanding our needs, finding the ways to satisfy them,

finding ways to resolve internal contradictions and conflicts,

allowing to enlarge our identity by new already appeared necessary qualities and mental parts and finding ways to do it,

finding "ecological" way to realize stressful and psychotraumatic energy,

and, finally, the manifestation of search activity as both an instrument for transforming undesirable situation, and, generally, a way to orient oneself in one’s life space,

in appropriate contexts and for appropriate levels of psyche development are necessary conditions for individual’s participation in the evolution of psyche. (It is clear that in case of deeper understanding of the regularities in the functioning of the psyche of organisms at the lower levels of development, this list will be enlarged.) Therefore, already known conditions of the genesis of psychosomatic diseases become special cases of a more general condition.

Besides, this hypothesis that interprets the psychological determination of body diseases genesis so widely, does not only define what diseases should be referred to psychosomatic diseases. On the one hand, it does not rely on any specific position about psychosomatics at all. On the other hand, it can be supported by both the followers of a psychosomatic paradigm who assume that all or many somatic diseases have psychological reasons (standing before somatic reasons or having equal meaning) and experts with opposite views who assume that mental reasons cannot have a substantial impact on the process of body diseases generation. Naturally, this creates an additional context for a dialogue between these paradigms.

We proceed with the list of useful effects caused by this representation of the nature of psychosomatics and can refer the possibility of formulating a general recommendation on keeping somatic health to them. We are healthy so far as our lifestyle enables us to fulfil our evolutionary process. Therefore, we have to solve two objectives throughout our lifetime: a) to be aware of "what our evolutional process" wants; b) to find a way to realize the vector of this process in our life. Therefore, we can conclude that not any development and not any search will contribute to health protection! Besides, our representation of the necessary vectors of our development may not coincide with a "view of nature" on them. (As we know from logic, a necessary condition is not always sufficient!)

The author thinks that the above assumptions of the evolutionally determined function and a condition of psychosomatogenesis activation, which coincides here with the genesis of somatic diseases in general, represent a concept that enables to look at the known things from another perspectives and creates contexts for both studying the unknown and uniting the efforts of specialists who have quite different views on the psychosomatic problem. Naturally, this concept works in the context of studying the nature of somatic diseases of all the representatives of the animal world.

Our concept enables to clarify the functions of psychodomatogenesis and the main conditions of its activation. Besides, it enables to look at the reference points in the research of its mechanism from a new perspective. The author admits that his representations of the general mechanism of (psycho)somatogenesis need to be further elaborated. However, he finds it proper to share them in this paper. In this context, it is logical to divide the question of the mechanism of psychosomatogenesis into two questions: "What part of a living organism "decides" that an individual participates or does not participate in the evolutional development of the psyche?" and "How does the mechanism of disease development activate itself (after it)?".

We assume that the second issue is more relevant for the present that the first issue. Therefore, we would rather start with it. In our opinion, it is quite logical to assume that we should look for a physiological mechanism that pursues the "interests" of the evolutional process through the development of diseases in something that relates to the work of a genetic apparatus. Nowadays, the importance of genetic expression in the functioning of organism in general is hardly doubted. Modern neurobiological research shows that gene expression depends on the mental state, while the mechanism of genetic expression influence the course of changes in brain and psyche [38; 47; 48]. We assume that an executive mechanism that switches on (after a certain signal) "the modes" of health and development of disease is activation or deactivation of a certain group of genes in somatic cells. These "pattern of health" and "pattern of disease" define how an entire organism and its different organs will respond to stress, infection etc., switch different physiological systems (regeneration, immune) on and off, activate different mechanisms that, in the long run, either maintain health or activate disease or just contribute to a disease.

What system defines whether one of these patterns should or should not be activated? Where should we look for it? Basing on the most general ideas, we can assume that first, neurons that respond to an individual. performing a behavioral or mental action that it has never performed before and even to an "evolutionally significant" action will emerge in some brain structures of higher animals in time (certainly, apart from the actions caused by learning during the development of an adult individual from a child individual. On the analogy with "neurons of novelty" and "neurons of equity" of a signal perceived. Second, the executive mechanism of a disease/health can respond to an activated process of genetic record in sex cells. Third, it is perfectly natural to assume that the above activation of genetic patterns is activated by certain proteins.

It goes without saying that the potentials of health protection will emerge if we really discover at least one of these physiological processes and find the ways to activate or imitate them! However, it is also quite clear that if the "goals" of a system do not coincide with the "goals" of its elements, a system may reveal a larger range of means to prevent an element from blocking the effects of instruments that realize its "goals"! We assume that finding ways to participate intentionally in the evolutional process of the psyche is a more guaranteed way to manage health!

We should note that irrespective of general executive mechanism of psychosomatogenesis under study, it is in any case a mechanism of mechanisms. Therefore, the specific mechanisms that have already been revealed by now should be its parts or its special cases. We also assume that the "goals" of various instruments of an evolutionary process should agree with each other, while specific mechanisms should cooperate. In our context, this primarily refers to the relations between the mechanisms of psychosomatogenesis and ageing including the two related hypotheses of the nature of oncology mentioned above. Taking into account that cancer formation is a growth in the number of cells uncontrolled by the process of ageing, which is not restrained (in a certain key moment) by "encouraging" the immunity, we have assumed in the first hypothesis that this process is a "rebellion" against the process of ageing, an attempt to substitute the ageing tissues by non-ageing [27; 28; 39], a "rebellion" against one of the crucial instruments of evolution at the level of an individual, which is a global tendency in an evolutionary process. However, we know that "rebels" die earlier. Evolution "has found a way" to turn an attempt to confront ageing into a life-threatening disease. In the second hypothesis, we have assumed that the physiological mechanism that underlies this "rebellion" is a mechanism of regeneration of earlier multicellular organisms activated during their injury (similar to the mechanism of stem cells work), which works incorrectly now [28]. We should note that now, this mechanism is likely to be activated in case of a psychological trauma, which is of great importance for highly developed organisms. (This hypothesis can be perfectly independent from the first one.) However, we assume that the lack of the evolution of this mechanism can also be explained by the "interests" of an evolutionary process. The above examples try to shed light on the nature of oncology. Besides, they are possibly indicative of a further elaboration of the issue of cooperation between various evolutionary instruments and the relations between the individual mechanisms of somatogenesis and a general mechanism.

The author understands that the concept is still at the initial stage of elaboration. It needs a clearer justification, empirical evidence and the solution of uncertainties left. Arguably, it has more questions than answers now. However, we know that proper setting of problems largely defines the success of research. It is probably the setting of the objectives of further research into the nature of psychosomatogenesis and means to protect health that will become its main achievement. Besides, we dare to think that possible objections to our position will lead to a discussion that would clarify the topic under study in general.
