Executive dysfunctions in childhood

Abstract

The term "executive function" is used as a common term that includes various complex cognitive processes. They are evolutionary features related to survival and are based on the ability to trace the current behaviour, to track the reactions of others and to achieve a change in behaviour according to the situation. Executive functions provide the ability to plan, organize, prioritize, focus on a specific task, use information and past experience when solving current problems.

The executive function is a "product of the coordinated work of various processes, to achieve a specific goal in a flexible manner". These complex processes are not separate structures but chains. Executive functions provide a buffer so as to rethink the current information, to create a plan and to choose a specific course of action.

Executive function disorders in children cause a wide range of symptoms. The term "executive function" includes a number of cognitive, emotional and behavioural difficulties.

Executive functions have been studied in various developmental disorders — developmental dyslexia, ADHD, autism spectrum disorders.

Deficits related to the development of the higher cortical functions and linguistic system lie at the core of executive dysfunctions in developmental dyslexia. This strengthens the opinion that executive dysfunctions have a secondary character and affect, above all, the abilities related to shifting, decision-making, planing, initiating and time management.

The main executive dysfunctions in children with ADHD are related to difficulties in task organization, estimating the duration of task performance, prioritizing and initiating, maintaining the focus, making efforts for processing, regulating the sleep-wake cycleand problems withemotional control, working memory, monitoring and regulation of the activity.

Children with Autistic spectrum disorders have significant difficulties in analytical thinking, in self-monitoring and in mental and behavioural flexibility. Behavioural problems are related to insufficient initiation of new actions or behaviours and a tendency to fix on a particular activity.

Despite the growing interest towards executive functions in various developmental disorders, there is still no convincing evidence for the presence of a specific executive profile for the different nosologies.

The therapy of executive dysfunctions in childhood requires interventions in three main directions — providing a protective environment for the child, therapy with the parents and therapy aimed at the children themselves.

In recent years there has been a consistent trend of an increase in the number of children with discrete, partial disorders, whose condition cannot be referred to a specific nosological unit, even though their social and school performance is problematic. These disorders affect their functioning at school and put them at risk of dropping out or developing emotional and behavioural disorders.

In recent decades environmental conditions and quality of life have changed dramatically, technology has violently invaded even the day care centers and kindergartens. Children grow in conditions of reduced or even no sensory and motor activity. These altered conditions lead to dissociation and functional immaturity of the cerebral cortex. In periods of age crises or increased functional loads, these dissociations limit the mental and social functioning of the child. The large-scale use of technologies even by very young children has changed the communication between children, which subsequently prevents the formation of their social skills. Future studies will provide answer to the question of whether these drastic changes affect the brain functioning and how.

Until recently, the executive dysfunctions in childhood were undeservedly neglected. Today it has already been proven that the development of these functions begins much earlier than we used to suppose. It has been proven that the executive functions in children have the same structure as in adults and influence their coping pattern and quality of life.

Neuroimaging techniques have allowed us to specify the localization of executive functions in the brain regions. Among the cortical postcentral regions, the most significant role in securing executive functions is played by the frontal and parietal regions located around the intraparietal sulcus (Sulcus intraparietalis). In M. Posner’s works [24; 25], these zones are regarded as key units in the brain organization, securing the shifting of visual attention. It is well-known that both the frontal and parietal regions of the cortex are activated in different situations that require the participation of selective attention mechanisms — this is what has produced the term "frontoparietal attention system" [8; 26]. The realization of executive functions is also carried out through the interaction between the structures of the limbic system — amygdala, hippocampus and hypothalamus — the basal ganglia and the mediodorsal nucleus of the thalamus.

Many studies have been conducted in an attempt to pinpoint the exact brain regions that lead to executive dysfunctions and that in turn has led to a huge amount of information which is often contradictory. A common hypothesis is that they are related to pathology in the prefrontal cerebral region [12; 13]. Various studies have shown that executive dysfunctions are determined by the interaction between the frontal cortex and other regions such as the basal ganglia and the cerebellum [4; 11]. Neuroimaging studies have shown that some functions are not localized in the prefrontal regions [6]. Studies with magnetic resonance have affirmed that there is a common network of regions in the frontal, parietal and occipital areas, thalamus and cerebellum [29]. This shows that executive functions are carried out by dynamic and flexible networks, which are characterized by integrativity and connectivity.

The term "executive function" is used as a common term that includes various complex cognitive processes. Defining them turns to be quite difficult due to the fact that executive functions do not constitute a unified conception. Executive functions are skills related to management of one’s own abilities and behaviour. They are an evolutionary feature related to survival and based on the ability to trace the current behaviour, to track the reactions of others and to achieve a change in behaviour according to the situation. In this sense, executive functions are directly related to our ability to apply the concept of significance of a given situation. They allow a quick response to the stimulus and this is based on past experience. They provide the ability to plan, organize, prioritize, focus on a specific task, use information and past experience when solving current problems.

In relation to the development of the views on working memory and attention, the concept of executive function gradually begins to clarify. The role they play gives us reason to talk about them as meta-procedures. In fact, they are the psychological means for transforming the knowledge and creating new meaningful contexts and mental spaces.

Three are the basic cognitive operations that are most commonly discussed as major constituents of executive functions:

• 

Storage and updating of significant information (updating);

• 

Selection of significant events and suppression of impulsive reactions (inhibition);

• 

Shifting to another cognitive task (shifting).

Although there is no common definition for executive functions, most researchers agree that they include inhibitory control, attention characteristics and information updates [27]. Executive functions and cognitive processes are connected in the so called metacognition-cognitive processes of higher order, they process and regulate information [28]. There is a presumption that megacognitive functions evolve through the feedback loops during various activities, or from the so-called metacognitive experience. Initially, the skills are implicit, but they gradually become accessible to the mind and become explicit. The term "executive functions" is an umbrella term to describe the processes in these adaptive reactions. They are related to the acquisition of new skills, planning and decision-making, correcting mistakes in the process of activity, initiating new actions, dealing with difficulties, conscious behaviour control, coping with impulsive reactions, etc.

The executive function is a "product of the coordinated work of various processes, to achieve a specific goal in a flexible manner" [10]. This flexible process coordination is related to the control systems. When these systems are compromised, the behaviour becomes uncontrolled and disinhibited, it is now regulated not by internal goals but by occasional external impulses, thus one loses both the ability to criticize one’s own actions and the ability to understand and correct one’s own mistakes.

Neuroimaging methods assess directly the neuronal basis of executive functions. These complex processes are chains, not separate structures. Executive functions provide a buffer so as to rethink the current information, to create a plan based on the available knowledge and to choose a specific course of action. Russell Barkley [2] believes that the ability to inhibit impulsive reactions is an essential aspect of executive functions, which in turn allows for further rethinking before the next action. Executive functions are often compared to a conductor of an orchestra, a manager of a company or a football coach.

The executive function is an umbrella term for many abilities, including: inhibitory response, cognitive flexibility, network of achievable goals, initiation of tasks, planning, organization and time management, abstract thinking and formation of concepts, working memory, attention control, emotional control and social behaviour, self-monitoring and regulation, metacognition.

Emotional control is connected with the ability to measure the emotional reaction and to change the response when the situation requires it.The ability to monitor the appropriate behaviour, to take into account the changes in behaviour and to meet the expectations is known as self-monitoring. The ability to handle information and to use it when necessary is related to the so-called working memory, which is one of the important executive functions.

According to Zelazo [30] the basic executive functions are: cognitive flexibility, inhibitory control, working memory, planning and self-awareness.

Each executive function is based on elements from the rest. Working memory functions based on the ability to manipulate with different parts of the incoming information. Mental flexibility is related to the ability to allocate attention depending on the requirements of the environment and the preliminary plan. Inhibitory control is related to the ability to prioritize and control impulsive actions. The ability to "stay focused" on a given task is the basis for the planning and performance of the task. The ability to plan includes skills to foresee the consequences, to preserve the goal and to develop a strategy for achieving it. Working memory allows to follow multi-step instructions and to use past experience.

The studies of executive functions in childhood have been neglected for many years unlike the studies of the same functions in adults. This is probably due to the statement of A. R. Luria (1973) that the functional maturity of the prefrontal cortex is reached around adolescence. A boost in the study of these functions is given by the use of neuroimaging techniques — they allow us to trace which brain regions are activated when performing certain tasks and what differences and similarities there are in the functioning of the child’s brain and the adult’s brain.

Children are not born with formed executive functions, but they are born with the potential to develop them. If their relations with the main carer and the adults do not provide them with a development environment, the formation of executive functions will be slowed down. The adverse conditions arising from negligence, abuse or violence may jeopardize certain brain structures and impede the development of executive functions.

Executive functions play a crucial role in the early stages of learning new skills. The more the skill is improved and perfected, the more the role of executive functions diminishes. Executive functions play a crucial role in the initial mastery of new skills, during the period when they are not automatically performed. On the other hand, the development of executive functions increases the ability of children to learn new skills, to act in a purposeful, organized and planned way. The flexibility of thinking and the ability to control one’s own thinking and behaviour increase. Inhibitory control is only one component of development, but it has a crucial role.

Executive functions are a set of opportunities that are fully achieved in maturity. Studies have shown that it is too risky to extrapolate the pattern of executive functions in adults to childhood, but this is still used. The effective executive functioning is a fact in adolescence, a time when neural systems of the frontal cortex have reached the ultimate degree of myelination, as in adults. There are executive functions that reach their maturity even at an earlier age. They are gradually acquired, which is associated with biological maturation and, accordingly, with the maturation of brain structures. This is a relatively long period of time and because of that they are at risk from the effects of the adverse conditions and, therefore, of inadequate formation. There is increasing evidence that frontal regions — responsible for executive functions in maturity — do not participate in the same way while these functions are still developing. Perhaps, much more extensive brain structures and neural networks are involved in this process. What is is particularly important here is that frontal regions are a regulator of the complex forms of mental activity. Frontal regions control thinking, volitional movements and are responsible for the creative approach in problem-solving. They also control emotions and participate in the formation of the child's personality. It is known that the transition from a senso-motor level to a higher one is associated with the maturation of prefontal regions, which is also associated with the improvement of working memory [3].

Three basic stages of development are important for the development of executive functions:

1)

From birth to 2 years of age. The foundation of executive functions is laid down in this period. Babies quickly learn new strategies such as organized research, purposeful behaviour, inhibitory response, memory and attention, nonverbal signals of social behaviour. This stage is connected with the presence of a "safe foundation", its availability and responsiveness allow the baby to explore the environment.

2)

From 6 to 9 years of age. Inhibitory control, planning skills, organizational strategies, concepts, working memory and creative problem-solving are built and formed during this period.

3)

From adolescence to early maturity. The coordination of working memory is formed, i.e. inhibitory control, complex planning and prognosis, visual working memory, verbal fluency, processing speed, self-monitoring, regulation and metacognition.

The analysis of specialized literature gives us reason to assume that executive functions in childhood include:

• 

Impulse control: This is the child's ability to stop and consider before acting.

• 

Emotional control: This is the child's ability to manage their emotions by focusing on the end result or goal. Emotional control and impulse control are closely related.

• 

Mental and cognitive flexibility: This is the child’s ability to apply new approaches when the plan fails and to shift from one activity to another. In difficult situations these children are rigid, stagnant, unaware of the existence of other solutions, difficult to change the plan, etc.

• 

Working memory: This is the child’s ability to keep information and use it to perform a current task. Children who have poor working memory skills have problems with multi-stage tasks.

• 

Self-monitoring: This is the child’s ability to monitor and evaluate how the task is performed the ability to evaluate whether strategies are working and to check their effectiveness.

• 

Planning and prioritizing: This is the child’s ability to realize the data necessary to achieve the goal, to arrange them and decide their order of importance. Kids with weak planning and prioritizing skills have difficulties in goal-setting.

• 

Initiation: This is the child’s ability to start performing the task; it is often associated with planning and prioritizing.

• 

Organization: This is the child’s ability to keep track of information, to organize themselves and the environment and to assess the negative consequences of disorganization.

Executive function disorders in children cause a wide range of symptoms. Most of them are related to the inability to initiate an action necessary for the performance of a task; a tendency to focus on details, or on the general but not on both — this is associated with attention deficits; inability to assess the duration of the task; slow and inaccurate or fast and impulsive performance of the task; inability to correct the plan of action; difficulties in starting up or shifting from one task to another; need for supervision, etc.

Executive dysfunction is a term that covers a wide range of cognitive, emotional and behavioural difficulties. Executive dysfunctions can be expressed as difficulties in organizing time, difficulty in handling and arranging materials and objects, difficulty in completing tasks, lack of or difficulty in thinking and behavioural flexibility, difficulties in maintaining active attention, etc.

Executive dysfunctions are not included in the DSM and ICD. Executive functions are present in many developmental disorders and complete the clinical picture without, however, being included as diagnostic criteria. Executive dysfunctions vary widely in different developmental disorders. These differences also define the impact they have on the functioning of the child and family. In most cases executive functions are not a diagnostic criterion, but their development and compensation of dysfunctions should be a part of the therapeutic strategies. Executive dysfunction is usually due to structural or functional frontal pathology. Despite the data provided from neuroimaging techniques, the attempt to connect executive functions only to prefrontal foci is not particularly convincing.

Meta-analyses prove that executive processes are mediated by networks involving numerous cortical regions, mostly posterior and prefrontal, and that the engaging of individual regions depends on the cognitive load. This point of view is an important conceptual change and significantly increases the challenges faced by neuroimaging studies. Instead of the relatively simplistic determination of a structure responsible for executive functions, the new approach requires understanding of the integration of a number of brain zones, their combination and integration. These combinations may depend not only on the requirements for performing tasks, but also on individual differences and past experience.

Developmental dyslexia

Deficits related to the development of the higher cortical functions and linguistic system lie at the core of executive dysfunctions in developmental dyslexia. This strengthens the opinion that executive dysfunctions have a secondary character and affect, above all, the abilities related to shifting, decision-making, planing, initiating and time management.

The established correlations suggest development of a therapeutic strategy based on a neuropsychological approach. The interventions are aimed at compensating the demonstrated deficits, starting subsequently from primary to secondary and tertiary disorders.

ADHD

It is indisputable that, to a large extent, ADHD is demonstrated by executive dysfunctions, but we cannot say that ADHD is synonymous with disexecutive syndrome. Children with ADHD have executive dysfunctions, but the criteria for its diagnosis are behavioural inadequacy and cognitive deficits. It is possible to identify executive dysfunctions, but they do not cover the criteria for ADHD. And that, to a large extent, increases the risk of subjectivity in diagnosis.

Studies on functioning in children with ADHD provide basis for distinguishing different types of executive dysfunctions: Difficulties in task-organization, difficulties in determining time for completion, prioritizing and initiation of the task. Difficulties in maintaining the focus, easy distraction. Difficulties in making efforts for processing, difficulties in meeting deadlines, problems in regulating the sleep-wake cycle. Difficulties in managing and modulating emotions. Difficulties in working memory and extraction of information. Difficulties in monitoring and regulation of activity, as well as in monitoring the context.

Based on the analysis of the data we can distinguish two types of ADHD — 31 children of combined type — hyperactive-impulsive, and 16 children of predominantly inattentive ADHD. 47 healthy controls were also tested.

The data from the study of the experimental group and their statistical processing identified a deficit of attention in all individuals included in the study. Such a deficiency is also present in those diagnosed with hyperactive-impulsive type of ADHD. The attention deficit correlates with difficulties in emotional control, inhibitory control, inability to use past experience, difficulty in extracting essential information and in time management. Non-purposeful activity has an impact on impulsiveness, time management, inhibitory and emotional control, use of inadequate strategies as well as inability to change strategies and to monitor context. Socially inadequate behaviour correlates with the inability to use past experience, with difficulties in emotional and inhibitory control, with manifestations of aggression, poor planning, difficulty in decision-making and initiating.

On the other hand, the inattentive ADHD subtype has revealed correlations between neuropsychological status and executive functions. Understanding complex instructions and verbal material correlates with inability to initiate and problems related to inhibitory control, decision-making and planning, time management. Social skills are affected by impaired inhibitory and emotional control, planning, difficulties in following instructions and learning difficulties.

The identified characteristics of functioning in children with ADHD suggest a therapeutic strategy aimed at changing the environment as well as interventions aimed at cognitive and executive functions — attention, inhibitory and emotional control, initiation, planning, working memory, self-monitoring and evaluation of the effectiveness of strategies.

Autistic spectrum

In recent years there has been considerable interest towards executive functions in autism spectrum disorders. Children with autism, even high-functioning, have significant disturbances in their ability to analytical thinking, self-monitoring and mental and behavioural flexibility. Behavioural problems are expressed by rigidity and perseveration and are interpreted as poor initiation of new behaviours or actions and a tendency to fixate on a particular activity. At the same time the ability to carry out routine actions can be preserved and expressed by a strong preference for routine actions — it is exactly what turns the behaviour into a stereotypical and ritualistic one.

Children with autistic spectrum disorders demonstrate difficulties in three main areas of executive functioning:

• 

Generation of new ideas and answers. They generate fewer new words and ideas as well as simpler answers.

• 

Planning. They show difficulties in tasks that require planning skills and these difficulties are maintained over time.

• 

Flexibility. Perseverative, stereotypical behaviour and motor acts are observed.

The poor performance of executive function tests in autism spectrum disorders is associated with stereotypical and rigid behaviour in everyday life, which is expressed in repetition of thought or action. There is still no consensus which aspects of executive functions are typical of autism spectrum disorders. This limits the potential of executive dysfunctions as a diagnostic marker for autism.

The therapeutic strategy is based on the concept of providing therapeutic assistance to the child and systematic consultation with their parents. Recognizing the emotions and social signs of others, planning skills, self-monitoring and building behavioural patterns in different social situations are in the center of the interventions with children.

Despite the growing interest towards executive dysfunctions in various developmental disorders, there is still no conclusive evidence of a specific executive profile for the different nosologies. This does not exclude the need for therapeutic work directed not only towards the child but also towards their parents and family.

Work with parents and family:

Resistance and protective behaviour of parents are caused by their experiences of anxiety, confusion and fear. Fear of facing their own experiences makes parents deny or ignore their child’s difficulties and mislabel them by using the wrong words such as "laziness", "lack of motivation", "inefficient school environment", etc. The reaction of denial is an expected mental response and precedes the gradual mobilization of the parents and their compliance with the instructions and requirements of the psychotherapeutic process.

Parental reactions to the child’s behaviour and school performance can be qualified in two opposite directions — hyper-diagnosing or neglecting of symptoms.

Executive functions are difficult to form and manage. They can be supported not only in a therapeutic environment, but in real life situations as well. Self-service, care for the living space and comfort of the others form and develop executive functions. It is extremely important that parents‘ requirements towards the child meet their real opportunities, determined by age criteria or certain pathological processes.

Often it is the parent’s condition that requires therapeutic intervention. Good therapeutic practices recommend Cognitive-Behavioural Psychotherapy. According to cognitive-behavioural psychotherapy, symptoms are cognitive phenomena, a distinct center of origin of symptomatology, while affective and somatic changes are a consequence of them. Cognitive-behavioural psychotherapy with parents of children with executive dysfunctions and developmental disorders aims at awareness of the problem, creation of a new framework of the traumatic event, re-conceptualization of life story, analysis of trans-generation models of affection and attitude towards the child, activation of resources for coping, etc. What can impede the therapeutic process with parents is the fact that they start the therapy because of their child and often are unable to address their own needs, outside their role as parents. Overcoming executive dysfunctions requires certain messages to parents and teachers which are based on the etiology and peculiarities of executive dysfunctions.

Work with children with executive dysfunctions

The therapy of executive functions in childhood requires interventions in three main directions — providing a protective environment for the child, work with the parents and interventions aimed at the children themselves. This ensures full development of the child’s potential and prevention of their isolation and neglecting. The effectiveness of therapeutic strategy in children with executive dysfunctions is determined by: identification of the problem and the child’s needs, verbalizing the possible solutions, formation of choice-making skills, strategy to implement the decision and evaluation of the result. The high variability of manifestations of executive dysfunctions requires application of techniques of different paradigms, this in turn defines the therapy as integrative and complex.

The therapeutic strategy in children with executive dysfunctions is built on the basis of the data derived from the intensive examination of each child and is consistent with the hierarchy of symptoms and relationship between primary defect and secondary disorders. It is necessary the impact to be directed towards the causes and the "lower" standing symptoms and it should be performed rhythmically and sustainably. Important components of therapeutic strategies for dealing with executive deficits are the interventions directed at child’s personality, they aim at acquiring new ways of action and behaviour or compensating for the deficits.

The therapeutic program is based on an individual approach to each child. This program depends on the objectives, the nature of difficulties, child’s cognitive and mental profile, parents’ readiness to get involved in the therapeutic process, the level of social support, etc.

An essential part of the therapeutic programs is the cognitive-behavioural approach that helps developing child’s skills to assess dysfunctional thoughts and patterns of behaviour, as well as the factors that support them. Due to the wide range of symptoms and the high variability of manifestations in different children, interventions require the involvement of a multidisciplinary team, with very clearly defined responsibilities of each member of the team. An essential part of the interventions is connected with recommendations to parents, teachers, peers, etc., for changing the environment of the child with executive dysfunctions and turning it into a protective one.

Аннотация

«Исполнительные (экзекутивные) функции» — общий термин для обозначения разнообразных сложных когнитивных процессов. Они развились в ходе эволюции для выживания и основываются на способности отслеживать текущее поведение, реакции окружающих и изменять поведение в зависимости от ситуации. Исполнительные функции дают возможность планировать, организовывать, расставлять приоритеты и концентрироваться на конкретной задаче, использовать информацию и прошлый опыт для решения проблем.

Исполнительная функция — это результат совместной скоординированной работы нескольких процессов, направленных на достижение цели посредством приспосабливания. Эти сложные процессы состоят во взаимосвязи и не представляют собой обособленных структур. Исполнительные функции выступают в роли буфера и позволяют обдумать имеющуюся на руках информацию, составить план поочередных действий и выбрать их дальнейшую последовательность.

Нарушения исполнительных функций в детском возрасте могут проявляться во множестве симптомов. Термин «исполнительная дисфункция» включает в себя ряд когнитивных, эмоциональных и поведенческих расстройств.

Исполнительные функции были исследованы в разных расстройствах развития: дислексии развития, СДВГ, аутистических нарушениях.

Проблемы, связанные с развитием высших корковых функций и языковой системы, лежат в основе нарушений исполнительных функций при дислексии развития. Это подтверждает мнение о том, что данные нарушения относятся к вторичным признакам и влияют, прежде всего, на способности, связанные с умением меняться, с принятием решений, планированием, инициативностью и эффективным использованием времени.

Основные нарушения исполнительных функций у детей с СДВГ связаны с трудностями в организации деятельности, в оценке времени, необходимого для выполнения той или иной задачи, в расстановке приоритетов и проявлении инициативы, удержании внимания, приложении усилий для совершения каких-либо действий, поддержании цикла сна-бодрствования. Отмечаются также проблемы с контролем над эмоциями, рабочей или оперативной памятью, контролем над своими действиями.

У детей с расстройствами аутистического спектра наблюдаются значительные трудности с аналитическим мышлением, самоконтролем, а также психической и поведенческой гибкостью. Поведенческие трудности связаны с недостаточной инициативностью по отношению к новым формам деятельности или поведения и склонностью к одному конкретному виду деятельности.

Несмотря на повышенный интерес к исполнительной дисфункции в разных нарушениях развития, не существует убедительных данных, подтверждающих существование определенного психологического профиля для разных нозологий.

Лечение исполнительных дисфункций в детском возрасте требует вмешательства по трем основным направлениям — это предоставление безопасной среды для ребенка, лечение вместе с родителями и лечение, направленное на самого ребенка.

В последние годы наблюдается стабильный рост числа детей с частичными дискретными нарушениями, состояние которых нельзя отнести к конкретной нозологии, несмотря на наличие проблем в социальной деятельности и успеваемости в школе. Подобные нарушения негативно влияют на учебную деятельность и могут привести к исключению ребенка из школы или развитию стойких эмоциональных и поведенческих расстройств.

В последние десятилетия состояние окружающей среды и уровень жизни претерпели значительные изменения, технологии вторглись даже в повседневную жизнь детских садов и групп продленного дня. Дети растут в условиях пониженной или полностью отсутствующей сенсорной и двигательной активности. Такие изменившиеся условия приводят к диссоциации и функциональной незрелости коры головного мозга. Во время возрастных кризисов или периодов повышенной функциональной нагрузки подобные диссоциации ограничивают психическое и социальное развитие ребенка. Широко распространенное использование технологий даже совсем маленькими детьми изменило коммуникацию между ними, что, в свою очередь, тормозит нормальное формирование социальных навыков. В будущих исследованиях будет найден ответ на вопрос о том, влияют ли такие серьезные перемены на работу мозга и каким именно образом они это делают.

До недавнего времени существование нарушений исполнительных функций в детском возрасте незаслуженно отвергалось. На сегодняшний день подтверждено, что развитие этих функций начинается намного раньше, чем предполагалось прежде. Было доказано, что исполнительные функции у детей обладают такой же структурой, что и у взрослых, и влияют на их качество жизни.

С помощью технологий нейровизуализации мы можем определить те области мозга, которые отвечают за исполнительные функции. Для обеспечения их безопасности в постцентральной части коры полушария головного мозга самую главную роль играют лобная и теменная области вокруг внутритеменной борозды большого мозга (Sulcus intraparietalis). В работах М. Познера [27; 28] эти области называют ключевыми для мозговой организации, обеспечивающими изменение (направленность) зрительного внимания. Известно, что лобная и теменная области коры головного мозга активизируются в разных ситуациях, для чего требуется участие механизмов избирательного внимания. Отсюда возник термин «система лобно-теменного внимания» [14; 29]. Осуществление исполнительных функций происходит за счет взаимодействия между структурами лимбической системы — миндалевидным телом, гиппокампом и гипоталамусом — базальными ядрами и медиодорсальным ядром таламуса.

В попытке выявить конкретную область мозга, отвечающую за нарушения исполнительных функций, было проведено множество исследований. В результате был собран колоссальный объем часто противоречивых данных. Согласно общему предположению, все экзекутивные нарушения связаны с патологией в префронтальной коре головного мозга [32; 33]. Многие исследования показали, что нарушения исполнительных функций определяются взаимодействием между лобной корой и другими областями мозга, такими как базальные ядра и мозжечок [8; 16]. Нейровизуальные исследования показали, что некоторые функции располагаются вне области префронтальной коры [10]. Исследования с использованием магнитного резонанса подтвердили существование единой сети из лобной, теменной, затылочной областей, таламуса и мозжечка [11]. Это доказывает, что исполнительные функции осуществляются с помощью динамических и гибких сетей, связанных между собой и действующих совместно.

«Исполнительные функции» — общий термин, включающий в себя разнообразные сложные когнитивные процессы. Определить его довольно затруднительно по причине того, что исполнительные функции — это умения, связанные с контролем человека над его способностями и поведением. Они развились в ходе эволюции для выживания и основываются на способности отслеживать текущее поведение, реакции окружающих и изменять поведение в зависимости от ситуации. В данном смысле исполнительные функции напрямую связаны со способностью человека оценивать важность и значимость ситуации, в которой он находится. Они позволяют быстро отреагировать на стимул или раздражитель на основании прошлого опыта. Они дают людям возможность планировать, организовывать, расставлять приоритеты и концентрироваться на конкретной задаче, использовать информацию и прошлый опыт для решения текущих проблем.

В связи с развитием взглядов на рабочую память и внимание понятие исполнительной функции начинает постепенно проясняться и формироваться. Из-за роли, которую они играют, мы можем рассматривать их как метапроцедуры. Действительно, они являются психологическим средством для трансформации знаний и создания новых смысловых контекстов и ментальных пространств.

Процессы, которые обычно относят к основным элементам исполнительных функций:

• 

хранение и обновление важной информации (updating);

• 

отбор важных событий и подавление импульсивных реакций (inhibition);

• 

переключение на другую когнитивную задачу (shifting).

Несмотря на то, что не существует устоявшегося определения исполнительных функций, многие исследователи согласны с тем, что они включают в себя ингибиторный контроль, характеристики внимания и актуализацию информации [30]. Исполнительные функции и когнитивные процессы связаны с метапознанием — когнитивными процессами высшего порядка, которые обрабатывают и контролируют информацию [31]. Существует предположение о том, что метакогнитивные функции развиваются через контуры обратной связи во время различных видов деятельности или через так называемый метакогнитивный опыт. Изначально эти умения имплицитны, но постепенно они становятся эксплицитными и доступными для сознания. Исполнительные функции — это общий термин, используемый для описания процессов в подобных адаптивных реакциях. Они связаны с приобретением новых навыков, планированием и принятием решения, исправлением ошибок в процессе деятельности, инициированием новых действий, умением справляться с трудностями и импульсивными реакциями, умением понять и исправить свои ошибки, осознанным контролем над поведением и импульсивными реакциями и т.д.

Исполнительная функция — это результат скоординированной работы нескольких процессов, направленных на достижение цели посредством гибкости [12]. Это гибкое взаимодействие процессов связано с системами контроля. Когда эти системы оказываются под угрозой, поведение становится неконтролируемым и расторможенным, теперь им управляют не внутренние цели, а случайные внешние импульсы, поэтому человек утрачивает способность оценивать свои поступки, осознавать и исправлять собственные ошибки.

С помощью методов нейровизуализации можно напрямую прояснить нейронную основу исполнительных функций. Эти сложные процессы взаимосвязаны и не представляют собой обособленных структур. Исполнительные функции выступают в роли буфера и позволяют обдумать имеющуюся актуальную информацию, составить план поочередных действий и выбрать способ их выполнения. Russell Barkley [6] убежден в том, что способность подавлять импульсивные реакции — это важнейший аспект исполнительных функций, который, в свою очередь, позволяет обдумать дальнейшее действие до начала последующего действия. Исполнительные функции нередко сравнивают с дирижером в оркестре, менеджером в компании или с тренером футбольной команды.

Исполнительная функция — общий термин-«зонтик» для множества способностей, в том числе таких как торможение, когнитивная гибкость, сеть достижимых целей, инициирование действий, планирование, организация и эффективное использование времени, абстрактное мышление и образование понятий, рабочая память, контроль над вниманием, эмоциями и социальным поведением, самоконтроль, метапознание.

Контроль над эмоциями связан со способностью измерять эмоциональные реакции и менять их, когда потребуется. Способность контролировать приемлемое поведение, учитывать изменения в поведении и соответствовать требованиям известна как самоконтроль. Умение получать информацию и использовать ее в случае необходимости связана с так называемой рабочей памятью, которая является одной из важнейших исполнительных функций.

По словам Zelazo [34], к числу базовых исполнительных функций относятся когнитивная гибкость, торможение, рабочая память, планирование и самосознание или самоосмысление.

Каждая исполнительная функция связана с другими. Функции рабочей памяти основываются на способности работать с разными частями поступающей информации. Ментальная гибкость связана с умением распределять внимание в зависимости от требований окружающей среди и предварительного плана. Ингибиторный контроль связан со способностью расставлять приоритеты и контролировать импульсивные действия. Умение концентрироваться на текущем задании является залогом его удачного планирования и поэтапного выполнения. Для планирования требуется умение предвидеть возможные последствия своих действий, придерживаться цели и развивать стратегию для ее достижения. С использованием рабочей памяти человек может следовать сложным инструкциям из нескольких пунктов и использовать прошлый опыт.

Исследованиям исполнительных функций у детей, в отличие от взрослых, не уделялось должного внимания. Отчасти так случилось из-за высказывания Александра Романовича Лурии (1973) о том, что функциональная зрелость префронтальной коры наступает ориентировочно в подростковом возрасте. Прорыв в исследованиях этих функций произошел благодаря использованию нейровизуальных технологий. Они позволяют увидеть, какие области мозга активизируются в момент выполнения определенных задач, какие сходства и различия можно обнаружить между мозгом ребенка и мозгом взрослого человека.

Дети не рождаются со сформировавшимися исполнительными функциями. Однако с самого рождения у них есть потенциал для их развития. Если через отношения со взрослыми (фигура привязанности) не удается организовать необходимые условия для развития, то процесс формирования исполнительных функций замедляется. Неблагоприятные условия, такие как безразличие, насилие и жестокость, могут поставить под угрозу отдельные структуры мозга ребенка и помешать нормальному развитию экзекутивных функций ребенка.

Исполнительные функции играют важную роль на ранних этапах осваивания новых навыков и умений. Чем лучше умение развито и отработано, тем меньшую роль играют исполнительные функции. Исполнительные функции играют важную роль в освоения новых навыков, в тот период, когда они еще не используются автоматически. С другой стороны, развитие исполнительных функций повышает способность детей к изучению новых навыков, умение действовать целенаправленно и организованно. Возрастает гибкость мышления и умение контролировать свой разум и поведение.

Исполнительные функции — это комплекс возможностей, который полностью формируется в зрелости. Исследования показали, что рискованно использовать для детей ту же модель, что и для взрослых, однако эта методика все равно применяется. Эффективная работа исполнительных функций наблюдается в подростковом возрасте, когда нейроны лобной коры покрываются миелином, как у взрослых. Некоторые исполнительные функции формируются в еще более раннем возрасте. Они оформляются постепенно, в процессе взросления человека и достижения зрелости структур головного мозга. Это занимает продолжительное время, и, кроме того, на нормальное развитие исполнительных функций могут повлиять неблагоприятные внешние условия. Появляется все больше свидетельств тому, что лобная область, которая отвечает за работу исполнительных функций, когда те формируются окончательно, функционирует иначе, чем когда эти функции находятся в процессе развития. Предположительно, в этот процесс вовлечены более обширные области мозга и нейронные сети. Здесь особенно важно отметить, что лобная область контролирует сложные формы психической деятельности. Она же контролирует мышление, отвечает за регуляцию произвольных движений и за развитие творческого подхода к решению проблем. Она также регулирует эмоции и участвует в формировании личности ребенка. Известно, что переход на следующий уровень после сенсомоторных реакций связан с развитием префронтальной области, которая, в свою очередь, связана с развитием рабочей памяти [7].

Развитие исполнительных функций состоит из трех важных этапов:

• 

С рождения до двух лет. «Фундамент» для исполнительных функций закладывается в этот период. Младенцы быстро осваивают целенаправленный поиск, ингибиторные реакции, невербальные сигналы социального поведения. На данном этапе младенец может исследовать окружающий мир благодаря постоянному присутствию и помощи матери или «надежной базы».

• 

С шести до девяти лет. В данном периоде происходит формирование ингибиторного контроля, навыков планирования, целенаправленных стратегий, понятий, рабочей памяти и творческого подхода к решению проблем.

• 

С подросткового возраста до ранней зрелости. Формируются процессы координации рабочей памяти: ингибиторного контроля, комплексного планирования и прогнозирования, зрительной памяти, вербальной беглости, скорости переработки информации, самоконтроля и метапознания.

Обзор литературы по теме исследования показывает, что в число исполнительных функций в детском возрасте входят:

• 

Импульс-контроль — это способность детей остановиться и обдумать свои дальнейшие действия.

• 

Контроль над эмоциями. Дети управляют своими эмоциями, когда сосредотачиваются на поставленных целях или на достижении результатов. Контроль над эмоциями тесно связан с импульс-контролем.

• 

Умственная и когнитивная гибкость. Это способность ребенка переключаться на другой подход в случае неудачи, с одного вида деятельности на другой. Нередко в затруднительных ситуациях дети не умеют проявлять гибкость, не знают о других подходах к решению проблемы, не могут их использовать и т.д.

• 

Рабочая память. Это способность ребенка удерживать в памяти информацию и использовать ее для решения текущей задачи. Дети, память которых недостаточно развита, не могут работать в режиме мультизадачности.

• 

Самооценивание — это умение оценить свои действия, их преимущества и эффективность после выполнения того или иного задания.

• 

Планирование и расстановка приоритетов. Это способность ребенка определить, какая информация ему потребуется для достижения поставленной цели, и структурировать ее по степени важности. Дети, у которых нет достаточных навыков планирования и расстановки приоритетов, испытывают трудности с формулированием и постановкой целей.

• 

Инициирование. Умение ребенка начать целенаправленное выполнение задания. Тесно связано с планированием и расстановкой приоритетов.

• 

Организация. Способность ребенка следить за перемещениями информации, организовывать самого себя и пространство вокруг, осознавать негативные последствия неорганизованности.

Нарушения исполнительных функций в детском возрасте могут проявляться множеством симптомов. Многие из них связаны с неумением инициировать действия, необходимые для выполнения того или иного задания, со склонностью концентрировать внимание или только на мелочах, или только на слишком общих вещах, но не том и другом одновременно (причиной этого является нехватка внимания), с невозможностью оценить временные затраты на выполнение задания, медленным и неточным или быстрым и импульсивным решением задач, невозможностью корректировать или полностью менять план действий, сложностями с инициированием или переключением с одной задачи на другую, необходимостью работать под контролем и т.д.

Термин «исполнительная дисфункция» включает в себя ряд когнитивных, эмоциональных и поведенческих расстройств. Исполнительная дисфункция может проявляться через трудности в эффективном распределении времени, удержании и структурировании информации и предметов, выполнении задач, мышлении и поведенческой гибкости, удержании внимания и т.д.

Исполнительная дисфункция не упоминается в Справочнике по диагностике и статистике психических расстройств (DSM — Diagnostic and Statistical Manual of Mental Disorders) или в Международной классификации болезней (ICD). Исполнительные дисфункции представлены в множестве нарушений развития и составляют клиническую картину, но не относятся к числу диагностических показателей. Исполнительная дисфункция по-разному проявляется в разных нарушениях развития. Степень проявления дисфункции определяет то, как нарушения развития повлияют на жизнь ребенка и всей семьи. В большинстве случаев исполнительные функции не относятся к числу диагностических показателей, но их развитие и устранение дисфункций должны быть в основе лечебной стратегии. Считается, что исполнительная дисфункция обычно развивается по причине структурной или функциональной патологии лобной области. Несмотря на данные нейровизуальных исследований, попытки привязать работу исполнительных функций к лобным долям мозга не выглядят достаточно убедительными.

Мета-анализ показывает, что исполнительные процессы связывают между собой многие области коры головного мозга, преимущественно затылочные и лобные. Отдельные области мозга включаются в работу при необходимости, в зависимости от степени когнитивной нагрузки. Такой подход меняет устоявшиеся взгляды и повышает сложность будущих нейровизуальных исследований. Несмотря на относительно простое определение структур, которые отвечают за исполнительные функции, в новом подходе требуется понимание того, как кооперируются и взаимодействуют между собой многие области мозга. Комбинации будут меняться в зависимости не только от поставленной задачи, но также от индивидуальных различий и прошлого опыта людей.

Особенности исполнительных функции у детей с дислексией развития

Дислексия развития связана с затруднениями в процессе переработки информации. Эти затруднения оказывают влияние на обучение грамоте и характеризуются несоответствием между ожидаемыми и реальными достижениями. Многие исследователи считают, что в основе дислексии лежит функциональная дезинтеграция между слуховой и зрительной переработкой вербальных стимулов. Вместе с затруднениями в чтении, письме и счете такие дети демонстрируют эмоциональные и поведенческие расстройства. Дефицит социальных умений связан с неспособностью оценивать мысли и настроения других, что связано с капацитетом. Кроме того у них есть заруднения в приспособлении к определенной социальной ситуации и неадекватное поведение.

Были исследованы 67 детей в возрасте 8—13 лет с жалобами на затруднения в обучении. Установлено, что между отдельными характеристиками внимания есть правопропорциональная положительная связь. Особенно выражен дефицит внимания в ситуациях, требующих эмоционального и ингибиторного контроля и использования прошлого опыта. Корреляционный анализ установил наличие значимой взаимосвязи между нейропсихологическим статусом, языковой системой и экзекутивными дисфункциями. Способность переключения коррелирует с социальной неадекватностью, инициацией, гнозисно-праксисными нарушениями.

Нарушения, связанные с развитием высших корковых функций и языковой системы, лежат в основе исполнительной дисфункции при дислексии развития. Это подтверждает мнение о том, что исполнительная дисфункция относится к вторичным признакам и, прежде всего, влияет на способность переключать внимание, принимать решения, планировать, инициировать действия и эффективно распределять время.

Установленные связи требуют разработки лечебной стратегии, основанной на нейропсихологическом подходе. Лечение должно быть направлено на последовательное устранение проявленных нарушений — от первичных к вторичным и третичным.

Особенности исполнительных функций у детей с СДВГ

Известно, что СДВГ преимущественно проявляется в исполнительной дисфункции, однако нельзя утверждать, что СДВГ и дерегулятивный синдром — это синонимичные понятия. У детей с СДВГ наблюдается исполнительная дисфункция, но критерием для ее диагностирования являются неадекватность поведения и когнитивные расстройства. Исполнительную дисфункцию можно выявить, но она не соотносится с критериями для выявления СДВГ. Кроме того возрастает риск проявления субъективности в постановке диагноза.

Исследования, посвященные детям с СДВГ, позволяют выделить разные виды исполнительной дисфункции: трудности в организации выполнения задач, определении времени на их выполнении, в расстановке приоритетов и инициировании действий по их выполнению; трудности в сохранении концентрации, быстрое переключение и отвлекаемость; трудности с приложением усилий для совершения действий; неумение укладываться в поставленные сроки; проблемы с поддержанием цикла сна-бодрствования; проблемы с контролем над эмоциями; проблемы с кратковременной (рабочей) памятью и извлечением информации; проблемы с контролем над контекстом ситуации и выбором дальнейших действий.

На основе анализа полученных данных можно выделить два типа СДВГ: из 47 детей — 31 ребенок с гиперактивным импульсивным смешанным типом и 16 детей с предоминантным невнимательным типом СДВГ. В исследования также участвовало 47 человек из контрольной группы.

Данные исследования по контрольной группе после статистической обработки позволили определить дефицит внимания у всех участников. Такое нарушение также наблюдается у тех, кому был поставлен диагноз «гиперактивный импульсивный тип СДВГ». Именно с дефицитом внимания связаны трудности с контролем над эмоциями, ингибиторным контролем, невозможностью применения прошлого опыта для решения текущих задач, трудности с извлечением необходимой информации и с эффективным распределением времени. Непроизвольные действия влияют на импульсивность, эффективное распределение времени, ингибиторный контроль и контроль над эмоциями, неадекватное поведение и невозможность менять модель поведения и следить за контекстом ситуаций. Социально неприемлемое поведение связано с невозможностью использовать прошлый опыт, проблемами с эмоциональным и ингибиторным контролем, с проявлениями агрессии, неэффективным планированием, неумением принимать решения и инициировать действия.

С другой стороны, невнимательный тип СДВГ позволил обнаружить связь между нейропсихологическим статусом и исполнительными функциями. Умение понимать сложные инструкции и вербальный материал связано с неспособностью инициировать действия и с проблемами с ингибиторным контролем, принятием решений и планированием, эффективным распределением времени. Проблемы с ингибиторным и эмоциональным контролем, умением планировать, следовать инструкциям и учиться влияют на социальные навыки человека.

Обнаруженные особенности среди детей с СДВГ показывают, что лечение должно быть направлено на изменение окружения, когнитивные и исполнительные функции — внимание, ингибиторный и эмоциональный контроль, инициирование действий, планирование, кратковременную память, самоконтроль и оценку эффективности действий.

Особенности исполнительных функций у детей с расстройствами аутистического спектра

В последние годы возрастает интерес к исполнительным функциям при расстройствах аутистического спектра. У детей с такими расстройствами наблюдаются проблемы с аналитическим мышлением, самоконтролем и психической и поведенческой гибкостью. Проблемы с поведением проявляются в отсутствии гибкости и персеверации мышления, которые связаны с низким уровнем инициирования новых видов деятельности или моделей поведения, а также со склонностью зацикливаться на одном определенном виде деятельности. При этом способность ребенка выполнять рутинные действия проявляется в его склонности к таким действиям, и поэтому поведение носит стереотипический и ритуальный характер.

Исследованы 26 детей в возрасте 8—14 лет с диагнозом синдром Аспергера.

У детей с расстройствами аутистического спектра возникают сложности в трех аспектах деятельности:

• 

Генерирование идей и решений. Они придумывают меньше новых слов и предлагают меньше идей, при этом их решения проблем проще, чем у сверстников.

• 

Планирование. Дети испытывают трудности при выполнении заданий, для которых требуются навыки планирования. С течением времени эти трудности сохраняются.

• 

Гибкость. Наблюдается стереотипное поведение и движения, а также персеверация мышления.

При расстройствах аутистического спектра наблюдается неэффективная работа исполнительных функций. Она проявляется в стереотипном поведении и отсутствии гибкости в повседневной жизни, что выражается в постоянном повторении мыслей и действий. Ученые не пришли к единому мнению о том, какие нарушения исполнительных функций являются характерными для расстройств аутистического спектра. Это ограничивает потенциал исполнительной дисфункции как критерия диагностики аутизма.

Интервенции основываются на предоставлении сопровождения ребенку и на систематических консультациях родителей. В основе работы с детьми лежит навык распознавания эмоций и стимулов от окружающих, навыки планирования, самоконтроля и выстраивание поведенческих моделей для разных социальных ситуаций.

Несмотря на повышенный интерес к изучению исполнительной дисфункции в разных нарушениях развития, достаточно убедительных данных, подтверждающих существование определенного экзекутивного профиля для разных нозологий, не существует. Однако это не исключает необходимости лечения, которое поможет не только самому ребенку, но и его семье.

Работа с родителями и с семьей

Отрицание и защитное поведение со стороны родителей продиктовано их опытом, связанным с тревожностью, растерянностью и страхом. Из-за страха столкнуться с собственными переживаниями родители отрицают или игнорируют проблемы ребенка и подменяют их такими понятиями, как «лень», «недостаток мотивации», «неэффективные условия для обучения в школе» и т.д. Реакция отрицания ожидаема с точки зрения психологии. После нее родители постепенно проникаются необходимостью психотерапевтического лечения и соблюдают рекомендации компетентных специалистов.

Реакции родителей на поведение ребенка и успеваемость в школе бывают двух видов: гипердиагностика и отрицание симптомов.

Исполнительные функции непросто сформировать и управлять ими. Однако поддерживать нормальную их работу можно не только с помощью психотерапии, но и в обычной жизни. Забота по отношению к себе, своему дому и окружающим поможет сформировать и развить исполнительные функции. Чрезвычайно важно, чтобы требования родителей к своему ребенку были выполнимыми и определялись в связи с его возрастом и отдельными патологическими процессами в организме.

Нередко терапия может требоваться именно родителям. В надлежащей терапевтической практике рекомендуют когнитивно-поведенческую психотерапию. В данной терапии симптомы — это познаваемые явления, на которых выстроена симптомотология, а эмоциональные и соматические изменения возникают вследствие симптомов. Когнитивно-поведенческая психотерапия для родителей и детей с исполнительной дисфункцией и нарушениями развития направлена на осознание проблемы, разработку концепции преодолении травмирующего события, переоценку биографии, анализ межпоколенческих моделей для выражения любви и отношения к ребенку, активацию ресурсов преодоления и т.д. Препятствовать процессу лечения детей совместно с родителями может тот факт, что из-за детей взрослые не могут представить себя вне роли родителей и не способны прислушаться к своим собственным потребностям. Для преодоления исполнительной дисфункции потребуется работа с родителями и преподавателями, повышение их осведомленности относительно этиологии и особенностей проявления исполнительной дисфункции.

Работа с детьми с исполнительной дисфункцией

Для эффективного лечения исполнительной дисфункции в детском возрасте необходимо работать в трех основных направлениях: обеспечить безопасную, защищенную среду для ребенка, работать с родителями и с самими детьми. Только в таком случае можно гарантировать развитие потенциала ребенка и предотвратить изоляцию и ощущение ненужности. Эффективность лечения исполнительной дисфункции у детей обеспечивают: определение проблемы и потребностей ребенка, обсуждение возможных решений проблемы, развитие навыков принятия решений и оценки результата собственных действий. Из-за множества проявлений исполнительной дисфункции необходимо использовать техники разных парадигм, из-за чего лечение можно определить как сложный, интегративный процесс.

Стратегия лечения ребенка с исполнительной дисфункцией основывается на данных, полученных в ходе тщательного обследования ребенка, и опирается на иерархию симптомов и взаимосвязи между первичными и вторичными нарушениями. Важно отметить, что нужно постоянно и систематически работать с причинами нарушений и симптомами, стоящими на «низшей» ступени. Важной частью лечения исполнительной дисфункции является работа с личностью ребенка, усвоение новых моделей действий и поведения и компенсация дефектов.

Программа лечения основывается на индивидуальном подходе к каждому ребенку. Она зависит от целей, характера нарушений, когнитивного и психического профиля ребенка, готовности родителей включиться в процесс лечения, уровень социальной поддержки и т.д.

Важной частью терапевтической программы является когнитивно-поведенческий подход, который помогает ребенку развить способность оценивать дисфункциональные мысли и модели поведения, а также факторы, влияющие на них. Из-за большого количества симптомов и их проявлений для лечения требуется междисциплинарная команда врачей с четко определенными обязанностями. Для повышения эффективности лечения необходимо проводить консультации и давать рекомендации родителям и учителям, чтобы изменить окружающую среду ребенка с исполнительной дисфункцией и сделать ее безопасной.
